AFFIDAVIT OF NON-OPERATION
I, the undersigned Director and President of a corporation under the corporate name
of TINA HAIR CORPORATION , a corporation duly organized and existing by virtue of the
laws of the Republic of the Philippines, with principal office at 466 Katipunan Street,
Labangon, Cebu City, Philippines, DO HEREBY CERTIFY:
1. That on April 16, 2018, TINA HAIR CORPORATION registered as a corporation by the
Securities and Exchange Commission

2. That since that date (April 16, 2018), TINA HAIR CORPORATION did not conduct any
transaction of its business, as a corporation, until December 1, 2018.

3. It is only in December 1, 2018, that the company started its operation as a corporation, and
performed business operation and transaction as a corporation under the name, TINA HAIR
CORPORATION.
4. As such, I am executing this affidavit to attest to the truth of the foregoing, and for purposes
of appropriate action by the Philippine Health Insurance.
