I am a legal permanent resident of the PHILIPPINE and I live at Block 936 Lot 37 Phase 9 Molave Drive
Heritage Homes Bry Loma De Gato Marilao Bulacan/Philippine .
I would like my husband Mr. Hewawasam Puwakpitiyage Chamara Niroshan Hewawasam and my two Step
Daughters Ms. Dinithi Sechira Oshanti Hewawasam and Ms. Manuthi Onethra Bimanthi Hewawasam to
come and visit me in Philippine from April 1 2019 to June 25 2019 because i’m giving birth of our baby.

My request is that they would be granted with a visa for this whole period. In which time I would be fully
responsible and taking care of them . They will also be resident at my home and following the expiration
of there visa.
