Sub: Request for Reference No. generation for Home Loan Part-payment via NEFT for Loan A/C

Dear Sir/Madam,

I am having a home loan running with your bank under loan a/c no.. Prior to Dec, 18 my loan EMI amount
was 17662/- which was exactly same as of my ECS mandate amount. But in Dec, 18 due to increase in loan
interest rates, EMI amount was increased to 17690/-. This resulted in non-presentation of ECS request by
ICICI to my bank, from which loan EMI was being deducted, for months Dec, 18 and Jan & Feb, 19. For
these three months I continued to make NEFT payments in loan account to clear my EMI’s.

Currently, from last 3 months, I am out of India and will remain so for next couple of months. To make
EMI ‘equal to or less than’ ECS mandate amount, I want to make part-payment in my loan account via
NEFT. As informed by customer care services, I must get NEFT payment service request number which I
have to mention in my NEFT payment remarks, otherwise NEFT payment will be rejected. I request you to
generate this ‘NEFT Service Request Number’ against my loan account for a part-payment of 50000/-.

I am authorizing Mr who is my younger brother to collect this Service Request number on my behalf. His
passport and mine copies are attached as an ID proofs with this request (scanned copy).
