DEED OF DONATION

KNOW ALL MEN BY THESE PRESENTS:

This DEED OF DONATION, made and executed by the Grade 9 Class S.Y. 2015-2017
represented by Mrs. CLARITA N. IDIA, hereinafter called the DONORS, in favor of
MANGALDAN NATIONAL HIGH SCHOOL, represented by DR. LEO E. BLAQUIR,
hereinafter called the DONEE,

WITNESSETH:

That the DONOR/S is/are the owners of a personal property particularly described as follows:

1) 1 set o 4f Gang Chair ₱ 6,500.00

That out of pure generosity and concern which the DONORS have for the cause of
education, the said DONOR by these presents does hereby give, transfers and convey unto the
DONEE, the above-described donation.

That DR. LEO E. BLAQUIR, representing the DONEE in his capacity as Principal III
does hereby accept this donation for and in behalf of the DONEE.

IN WITNESS WHEREOF, the DONOR and for and behalf of DONEE have hereunto
signed this Deed of Donation this 26th day of April, 2017, in Mangaldan, Pangasinan,
Philippines.
