Closing-the-Gap Lesson Plan

Activity: Check In Check Out (CICO)

Grade(s): IEP students who have been late 5 times in a month

Learning Objective(s) (aligns with competency):

1. Students will create short term goals for the school day/week.

2. Students will enhance interpersonal relationships with peers and teachers.

3. Students’ attendance and behavior will improve.

Materials: Check in Check out worksheet, CICO checklist, Take Home CICO parent form

Procedure: Each morning the student will check in with counselor. Counselor will assess how well-prepared the student

is for that school day. They will receive their sheet for the day and create short-term goals for that school day. After each

period the student will assess their own behavior and receive feedback from their teacher. The student will then bring the

sheet to the counselor at the end of the day to discuss the feedback and any other pressing matter. The student will then

receive a Home Check-In sheet they will bring home and get signed by a parent or guardian.

Plan for Evaluation: How will each of the following be collected?

Process Data: Students with IEPs who have missed school five times in the last month will complete the CICO worksheet each

day for 3 weeks.

Perception Data: 90% of participants will be able to identify a supportive relationship within the school and will be able to identify

positive behavior in the classroom. A pre and post test will be administered.

Outcome Data: Chronic absenteeism for students with IEPs will decrease by 2% over the next school year, moving from 87%

to 85%.

Follow-Up: After three weeks, the counselor will continue to check in with these students on a semi-regular basis and

provide extra support, such as study skills, time management skills, or other resources the students may need based on

the results of the CICO program.
