CONSENT FORM

I agree to participate in the study conducted by Allyza Mae Ting and Johnnie Manza,
students from the Information Technology of Pamantasan ng Lungsod ng Valenzuela.

I understand that participating in this usability study is voluntary and I agree to raise any
concerns or areas of discomfort during the session with the project proponents, above
mentioned. All data collected will be anonymous and my identity will be protected to the
extent permitted by the law, and that no data will be associated to any particular
individual o me.

I have read and understood the information on the form, and all inquiries prior to
answering this test were covered.

I hereby give my permission for today’s usability session to be photographed and or

videotaped for documentation purposes, thus it shall only be used internally within the
academe.

I am free to withdraw from the study at any time during the experiment if conditions are
not met, and I understand that there are no risks involved in participating in this study,
nor there any immediate benefits.
