CONTRACT OF RENT OF MOTOR VEHICLE

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF RENT, made and entered into this 11th day of September 2017 at

Muntinlupa City by and between:

RDMC TRANSPORT SERVICE represented by MARIBETH A. CASANOVA, of legal age, married,

Filipino and with residence and business address at Blk. 80, Lot 24 Katarungan Village, Poblacion,
Muntinlupa City, herein referred to as the FIRST PARTY,

and

BUCOR-PERONNEL TRAINING SCHOOL, represented by THEODORE R. PEREZ, also of legal age,

single, with business address at the Bureau of Corrections, NBP Reservation, Brgy. Poblacion,
Muntinlupa City, hereinafter referred to as the SECOND PARTY.

WITNESSETH that

WHEREAS, the SECOND PARTY hereby accepts to rent from the FIRST PARTY one (1) unit of
CROSS WIND VAN bearing plate number ACZ 2429, subject of the following terms and conditions, to
wit:

1.That said vehicle will be used to transport BuCor employees from the New Bilibid Prison,
Muntinlupa City to the Developent Academy of the Philippines (DAP) Conference Center, Tagaytay City
on September 12, 2017 and to fetch them from DAP Conference Center Tagaytay City to New Bilibid
Prison, Muntinlupa City on September 15, 2017;

2. That the SECOND PARTY shall pay the FIRST PARTY the amount of Nine Thousand pesos
(Php.9,000.00) representing the payment for the transport service;

3. That the fuel and toll fee for the said trips shall be borne by the FIRST PARTY.

In WITNESS WHEREOF, the parties hereunto set their hands this 11th day of September 2017 at
Muntinlupa City, Philippines.
