AFFIDAVIT OF LOSS

I, MARANATHA KRISTAL PORTAL, of legal age, Filipino, single

and a resident of Sabayle St., Iligan City, after having been duly sworn to an
oath in accordance with law, hereby depose and say, THAT:

1. I am a holder of IVT ID/LICENSE

2. Sometime on May 23, 2017, the City of Marawi experienced a

confrontation between the Maute Group and the Philippine Army
and due to that siege, we were not able to packed our things
properly including the aforementioned IVT ID/LICENSE and all
my other important documents and despite deliberate effort to
locate it, the same could no longer be found, hence, considered
lost;

3. I am executing this affidavit to attest to the truth and veracity of

the foregoing facts of my own personal knowledge and belief and
to attest to the said fact of loss and for the renewal of the said IVT
ID/LICENSE;

AFFIANT FURTHER SAYETH NAUGHT.
