    Observation date : june 26th 2014 (wk 26)

    Participants : Cox & Avin

    Area inspected : VT/UT & Floor Scanning of T-741A floor

    Observations :

    Permits ; workpermit : OK Confined space permit : OK

    Fresh Air supply : Forced ( Ventilation was very good ; 1 high performance airmover and 1 airblower)

    Confined space entry form : you had to sign in before entering and sign out after coming out of the tank.

    Work Coordination : Very Good ; Irem was working on the tank and we were working in the tank.

    Lighting : Very Good ( Stream lights with a ground fault interruptor)

    Cleaning : We got help from the painting crew (who were busy with outside of the tank) do clean
    the tank from all the rust-flakes that had fallen from the roof on the tank bottom , in a very high tempo .

    Fresh Air Break : we took a 10-15 min. break after 40-45 min. of working in the tank , because the tank
    was ventilated very good.

    Conclusion:

    Due to a good work planning and work coordination we succeeded to finish this job within 4 hours in a very safe way.
