Waiver/Request for On the-Job Training

Sir:

The undersigned parent/guardian requests that his/her daughter/son,

_______________________________________ presently undergoing On-the-Job
Training at the __________________________________________________ be
permitted to undergo OJT in the LGU PINABACDAO to the following conditions:

1. The training shall be in no way in an employer employee relationship;

2. No compensation shall be given to the trainee for the work done during his/her
training; however, the LGU PINABACDAO may opt to grant incentives in the
form of daily allowance and other benefits;
3. The LGU PINABACDAO shall not be liable for any injury sustained or sickness
contracted arising from the training;
4. The LGU PINABACDAO shall not be obliged to hire the trainee after the
completion of his/her training;
5. The parent/guardian shall be responsible for the personal conduct of the trainee
and shall see to it that the trainee:
a. Attend OJT and other related functions regularly and punctually;
b. Follow strictly the LGU PINABACDAO rules and regulations;
c. Refrain from participating directly and indirectly in the labor union
activities and labor problem.
6. EVSU and its officials, and LGU PINABACDAO and its officials are free and
harmless from any civil and criminal liability that may fall upon the students while
inside the work premises caused by incidents or accidents.
