ANNEX “C”

CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

This Agreement made and entered into at Makati this 17th day of January2013 by and
between JUAN DELA CRUZ, of legal age, married to JANNA DELA CRUZ, (LESSOR)
and resident of Makati City, and JACOB GOMBURZA, of legal age, single and resident of
Quezon City (LESSEE), WITNESSETH that:

1. In consideration of a monthly rental of TEN THOUSAND PESOS (P10,000.00)

and the covenants made below, the LESSOR hereby LEASES to the LESSEE an apartment
located at 199 San Antonio Village, Makati City covered by Tax Declaration No. 001
(Makati City Assessor’s Office) for a period of TWELVE (12) MONTHS from signing of
this contract.

2. The LESSEE covenants, as follows:

2.1. To pay the rentals on or before the fifth day of each month, without need of
demand at the residence of LESSOR;

2.2. To keep the premises in good and habitable condition, making the necessary
repairs and painting inside and outside the house;

2.3. Not to make major alterations and improvements without the written consent
of the LESSOR and in the event of such unauthorized major alterations and
improvements, surrendering ownership over such improvements and alterations to the
LESSOR upon expiration of this lease;

IN WITNESS WHEREOF, the parties have signed this contract on the date and the
place first mentioned.

ACKNOWLEDGEMENT

known to me to be the same persons who executed the foregoing instrument, denominated
as a Contract of Lease consisting of two pages, signed on each and every page by the
parties and their instrumental witnesses, having acknowledged the same before me as their
own free and voluntary act and deed.
