CHECKLIST OF REQUIREMENTS FOR ASSOCIATE MEMBERSHIP

APPLICATION

Criteria A:

1. Letter of intent

2. Fully accomplished NRCP TCDS F1 Form

3. Two copies of publications within the last five (5) years (full paper with complete
details such as volume, number, pages, and date of publication)

4. Certification of research undertakings for the past three (3) years, containing the titles
of past and/or on-going researches, and signed by the Head of the Agency/Research
Director

5. Copy of abstracts (for completed) or progress reports (for on-going) of the research
titles included in No. 4 requirement

Criteria B:

1. Letter of intent

2. Fully accomplished NRCP TCDS F1 Form

3. Certification of research undertakings for the past three (3) years, containing the titles
of past and/or on-going researches, and signed by the Head of the Agency/Research
Director

4. Copy of abstracts (for completed) or progress reports (for on-going) of the research
titles included in No. 3 requirement

Note: Only fully accomplished forms shall be accepted.
