    Kindergarten Round-Up will take place at the Wasatch County Health Department in Heber City on April 17th, 18th, and 19th. On the 17th, registration will be for students at JR Smith and Heber Valley & Midway Elementary Schools. On the 18th, registration will be for Daniels Canyon & Old Mill Elementary Schools. The 19th is a make-up day. Parents should register online starting March 15th, print and complete the health form, and bring their kindergarten student plus proof of ID, residency, birth certificate, and immunization records.

    1. Register Online beginning  Your Kindergarten student!
    March 15th at (must be 5 by, Sep. 1st, 2019)
    sisweb.wasatch.edu/Students/  Driver’s License/Other Legal ID
    Register/Start  Proof of Residency*
    2. Print off and complete the  Child’s Birth Certificate
    Health Form  Immunization Record
    3. Come to Kindergarten Round-  Health Form
    Up!  Any legal custodial document

    *Proof of Residency: Legal ID plus utility bill, current vehicle registration, voter registration, payroll
    stub, bank statement, copy of most recent mortgage payment, or copy of lease AND record of most recent
    rent payment
