Sub: Request for extension of my leave.

Dear Sir/Madam,

Myself Amjuri Prasad and I am working as fabricator in HINO department ,my code
number is 29535.I am currently on approved annual leave from the date of 03 Feb 2019
to 04 March 2019 but due to some emergency situation at home, I have been compelled
to extend my leave for 12 days. Hence I humbly request you to please approve to
extend my annual leave up to 16 March 2019.I apologize the inconvenience caused. I
am attaching a copy of my return ticket. (I informed to Mr.Muthaiah Raman sir)
