Campaign: PruBSN Setia with Doctor2U for RM250 E-voucher

TERMS & CONDITIONS

1. Campaign Criteria:

Participation in PruBSN Setia plan with minimum annual* contribution of RM3,000, with
payment method via credit / debit card.

*Note: If payment frequency is in monthly mode, then 2 monthly contributions equivalent needs
to be paid up front by 30th September 2018.

2. Campaign Gift (“gift”): Doctor2U RM250 E-voucher. Limited to one (1) gift per one (1) PruBSN
Setia certificate participated.

3. Campaign period: 16th July 2018 – 30th September 2018

4. You will be eligible to receive the gift if:

(i) You sign up for PruBSN Setia meeting the Campaign Criteria during the Campaign Period;
AND
(ii) Your Proposal is approved and incepted by 31st October 2018; AND
(iii) You provided a valid mobile phone number in the proposal.

5. PruBSN will send the gift to you via SMS, after the campaign period to the mobile phone number
provided in your proposal. Any invalid mobile phone number given may cause your gift to be
forfeited subject to PruBSN’s sole and absolute discretion.

6. If the gift is stolen or lost for any reason or under any circumstances, no replacement will be
provided by PruBSN as all rights, interests, title and benefits to the gifts would have passed to
the eligible certificate holders at the time of delivery. Gifts are deemed delivered upon PruBSN
having sent out the gift.

7. The gift is not exchangeable nor convertible into cash.

8. The gift code is for the value of RM250 only. Any redemption above RM250 will require
additional top-up from customer. However, there will be no balance of cash payable from either
PruBSN or Doctor2U if any redemption of services from Doctor2U is below the value of the gift.

9. PruBSN has the absolute discretion and right to decline the entitlement for the gift in the event
these terms and conditions are not fulfilled.

10. For all intents and purposes, the entitlement for the gift shall not in any event be construed as a
variation to the terms and conditions under the certificate for PruBSN Setia plan.

11. PruBSN shall have the absolute discretion and right to change/substitute the gift with other
offers of similar value without prior notice to the eligible certificate owner. No appeal(s) will be
entertained.

12. PruBSN makes no representation, warranty or undertaking as to the quality or fitness with
respect to Doctor2U, which is product of BP Healthcare Group. PruBSN shall not be liable to the
eligible certificate holder, or any other third party(ies) including any transferees of Doctor2U
 services with respect to any loss, damage, expense, suit, directly or indirectly, arising from or in
connection with their usage of Doctor2U under the BP Healthcare Group.

13. If there is another similar PruBSN Setia campaign running concurrently with this campaign, the
eligible customers will be given only one gift that has the higher/highest value.
