Based on regular EMB evaluation activity you failed to submit your Self-Monitoring Report (SMR) since 2015. Pursuant to Revised Procedural Manual for Department Administrative Order No. 30 Series of 2003, Item 33 (c}, it is provided that: “In case of violation of ECC conditions...the sum of P50, 000.00 is set as the maximum amount of fine per violation and the violation of one condition in the ECC is an offense separate and distinct from the violation of another condition.” In view of the foregoing, you are further directed to submit a verified written explanation within a non- extendible period of fifteen (15) days from receipt of this Notice why no Order should be issued directing you to discontinue the project operation and/or penalized under existing laws, rules, and regulations.
