Part A. PROFILE
Figure 1. In terms of age, 80.43% belonged to age 11-12, and 19.57 belonged to age 13-14, generally
this implies that the age of the grade 6 students is on its appropriate or suitable for grade.

Figure 2. In terms of sex, 58.70% are female while 41.30% are male. It can be deduced that the class is
female-dominated.

Figure 4. Out of 46, 45 or 97.83% has a heart rate of 60-100 bpm which is normal for their age, while
one or 2.17% manifested abnormal heart rate of 100 beats per minute and above termed as tachycardia.
It can be deduced that majority of the students have no abnormality in pulse rate. However the students
whose findings were abnormal must be subjected to further diagnosis to identify underlying condition.

Figure 5. In terms of Respiratory rate, 31 students or 67.39% has an RR of 16-25 cpm, 15 or 32.61% has
an RR of 25 cycle per minute and nobody has an RR of 16 cpm below. This implies that majority of the
students has a normal respiratory rate while few has an abnormal RR, the abnormal RR or medically
termed tachypnea needs further examination as this vital sign is only an indicator that something in the
body is not doing its function well.

Figure 6. In terms of blood pressure, 44 or 95.65% has a BP of 110/90 and 2 or 4.35% has a BP of
<90/70 below. This implies that most students are having normal blood pressure and only few deviated
from normal. Blood pressure varies in each individual according to age, but the question whether what is
standard blood pressure according to age is provided by nursing references.

Figure 7
Figure 7. Majority belonged to the normal BMI with 31 or 67.39%, while 15 or 32,61% has a BMI
below normal. This implies that not all of the grade 6 students has a healthy nutritional status, and
therefore needs improvement.

PART D. MEDICAL HISTORY

Figure 8. Out of 46 students, 4 or 8.70% has allergy, 3 or 6.52% has fainting, then both convulsion and
heart ailments has only 2 or 4.35%, lastly 1 student or 2.17% have bleeding ailments. This implies that
26.09% or ¼ of all the students has medical problem and suggest for immediate action especially on
allergy having the highest percentage.

Figure 10
Figure 10. In terms of change of toothbrush in a year, it is recommended by the dentists that toothbrush
should be changed in every 3 months, thus 4x a year is enough. The table shows that mainstream of
students or 23.91% change their toothbrush 2x a year, 9 or 19.57%-3x a year, 8 or 17.39%-5x a year, 7
or 15.22% of students change toothbrush once a year, 5 or 10.87%-12x a year, and both 4x a year and

implies that majority of the students don’t know the correct practice of dental hygiene or has a
knowledge deficit about it.
