ข้อแนะนา - ภายหลังหยุดยา ควรเจาะ aPTT, aPTT ratio ก่อนเริ่มยาครั้งใหม่
ข้อแนะนา - วิธีการเขียนคาสั่งการรักษาในยาที่มีความเสี่ยงสูง (High Alert Drug) เพื่อความปลอดภัยสูงสุดแก่ผู้ป่วย ดังนี้ Heparin 2 ml (10,000 units) ผสมใน
NSS 100 ml (ความเข้มข้นเท่ากับ Heparin 100 unit ต่อ NSS 1 ml) iv drip rate 10 ml/hr (ผู้ป่วยได้รับ Heparin 1,000 unit/hr)
หมายเหตุ - การตรวจติดตามหลังจากที่ค่า aPTT ratio คงที่ติดต่อกัน 2 ครั้ง ให้ติดตามทุก 24 ชั่วโมง หรือ วันละ 1 ครั้ง
หมายเหตุ - ควรติดตามค่า CBC อย่างน้อย 2 ครั้งต่อสัปดาห์ เพื่อติดตามระดับ Hemoglobin และ Platelet
