AUTHORIZATION LETTER

my RELATION, NAME, also a resident of ADDRESS, to act as my true and
lawful representative in transactions with PLDT with regard to the application for
and/or upgrade to PLDT Home Fibr Plan XXXX of our current PLDT landline
under my name, with account number XXXXXXXXX and with landline number
XX-XXX-XXXX. This authorization shall include the authority to submit all
requirements and receive or pick up all documents and items that may be released
by PLDT in relation to the said transaction.
