SUBJECT: INSPECTION REPORT
State Law requires periodic inspections of county jal facilities (VTCA, Local Government Code,
Chapter 351, VICA, Government Code, Chapter 11; Chapter 297.8, Texas Commission on Jail
Standards).
The facility was inspected on the date(s) indicated above, and it was determined that
deficiencies exist. You are urged: (1) to give these areas of noncompliance your serious
‘and immediate consideration: and (2) to promptly initiate and complete appropriate
corrective measures, The Commission is available to discuss or assist you with the
appropriate corrective measures required,
Failure to initiate and complete corrective measures following receipt of the Notice of
Noncompliance may result in the issuance of a Remedial Order (Chapter 207.8, et sea.)

noted and upon review of this report by the Executive Director of the Texas Commission on
Jail Standards, a certificate of Compliance may be issued per the requirements of VTCA,
Chapter 511 and Texas Minimum Jail Standards.

While reviewing screening forms, it was determined that four (4) files did not contain
‘documentation o verify thatthe magistrate was notified within 12 hours as required by
‘Article 16.22 of he CCP.
Screening _Insinment. An approved mental disabies/suicde prevention screening
instrument shall be completed immediately onal inmates admited.
While reviewing screening forms, it was determined that seven(7) files were not
completed in its entirety. The jail staf falled to notate the time the screening took pace,
there were no comments when answering “yes" to questions, did not answer all ofthe
Auestions on the form and they falled to document notifications to the magistrate,
‘mental health, medical, andlor supervisor when required.
‘Observation shallbe performed a east every 30 minutes in areas wher inmates known tobe
_assautve, potentaly suicidal, mental i, oF who have demonstrated bizare behavior ae
confined.
‘During tho review of the 30 minute face-toface observations, it was determined the jll
staff exceeded the 30 minutes required by minimum standards by 1 minute up to 56
minutes.
