AFFIDAVIT OF ATTACHMENT OF SIDECAR

I, LUIS GONZALES, of legal age, a resident of Capirpiriwan, Cordon, Isabela and a

mechanic, after having been duly sworn to in accordance with law, do hereby depose and say:
1. That a motorcycle with Plate Number MC BG 6536 owned by NAPOLEON P.
ABAYA SR. of Quirino, Cordon, Isabela was brought to my shop for the
attachment of a side car;
2. That the attachment thereof takes into consideration the safety of all passengers and
I personally assume all the liabilities in case of any findings of negligence in the
actual attachment of the side car to the motorcycle;
3. That I, being the mechanic, assure the roadworthiness and safety of a unit as a
whole;
4. That the attachment of the said side car has been completed on November 5, 2018.

AND

I, NAPOLEON P. ABAYA SR., being the owner of the subject motorcycle express
my full satisfaction on the attachment of the side car.
That we are executing this affidavit to attest to the truth of the foregoing instrument
with the best of our knowledge and without mental reservation or evasion.
