CORRECTION OF ONLINE SIWES FORM

I Onyekaba Onyinyechukwu Anastestia (matriculation no 140401056), in an attempt to
complete the SIWES online form so as to meet with the stipulated deadline that was later
postponed, made a wrong input of the company name Federal Institute of Industrial Research,
Oshodi (FIIRO). I hereby humbly ask for a correction of the company name to Works and
Physical planning department University of lagos as well as correction of Training Supervisor
Mr. Olabode Oladipo.

I have attached a copy of the SIWES online form with the correction to be made written on it.
