I would like to express my intention to enter into a Contract of Service reporting

Reference and Fees based on the prevailing market rates as stated under Section
8.0 of Civil Service Commission – Commission on Audit – Department of Budget

no employer-employee relationship, and subject to the submission of
accomplishment reports based on the detailed Terms of Reference as attached.

3. PhilGeps registration Number, screenshot attached;
4. Copy of BIR Certifcate of Registration (original to be presented for
comparison purposes);
5. Certificate of relevant trainings as applicable.
