AFFIDAVIT

I, JOLLIE B. INASAL, of legal age, married, Filipino and a resident of San Gabriel
Village, Tuguegarao City, Cagayan, after having been duly sworn to in accordance with
law, do hereby depose and state;

That I am the store manager of Jollibee Tanza;

That on evening of March 10, 2019 until the morning of March 11, 2019, I was on
duty to look over the operations of the store during the closing shift;

That as such, I was in-charge of the time log of the service crews, cashiers and
kitchen staff who are on duty and will report for duty for the opening shift from 5:00 am
to 1:00 pm, mid-opening shift from 8:00 am to 4:00 pm, mid shift from 11:00 pm to 7:00
pm, mid closing shift from 2:00 pm to 10:00 pm and closing shift from 8:00 pm to 5:00 am
and to supervise the cashiers and the counter;

That on the said day at about 7:45 in the evening, I saw LITO LAPID III, who was
one of the cashiers assigned to report for the closing shift, in fact he had logged in for his
duty and as proof is the photocopy of the daily time record of LITO LAPID III and the
logbook kept by the security guard on duty which are hereto attached as ANNEX “A”;

Furthermore, as the manager on duty, I directly supervised and oversaw the

transactions and customer-relations of the cashiers;

That during the entire duration of the shift of LITO LAPID III, he never left his
post except for brief calls of nature and that he never left the store premises until the end
of his shift, as proof is a video clip derived from the CCTV footage from 7:00 pm of March
10, 2019 to 7 am of March 11, 2019;

That I am executing this affidavit to attest to the veracity of the foregoing

statements in utmost good faith, freely and voluntarily without being forced, coerced nor
intimidated.
