Referee Evaluation Form

Dear Coaches and/or Assistant Coaches,

Thank you for taking the time to assist our referee program in producing the most educated and professional officials possible.
We appreciate both your positive feedback as well as items to improve upon. Please use the form below to help evaluate both
your center and assistant referees. Please fill out this form as clearly as possible, using very specific details. Evaluations will
remain anonymous to the referees unless otherwise requested by the coach or assignor. Please be mindful, the majority of our
officials are 14 -18 years of age and are often still learning. It takes a great amount of knowledge of the game, self-confidence,
and reliability, among other important attributes, to step up to the job of referee. These evaluations will be used to give
feedback and constructive criticism to referees to help them to learn and better themselves. Overly nasty or otherwise
derogatory comments will not be tolerated. Hard copies of this form and envelopes can be found under the concessions roof.

This form can be handed in to the referee assignor, in one of several ways including:

1. Hard copy handed directly to the referee assignor, Amy Lemmon, in a sealed envelope.
2. Digital copy emailed directly to Referee Assignor, Amy Lemmon BaysaReferee@gmail.com
3. Hard copy placed in a sealed envelope, marked “Attention Amy Lemmon”, and placed in the drop box under the
concessions roof.
4. Digital copy at www.baysa-soccer.org > Coach/Referee Evaluation submitted directly from site by following
instructions online.

1= poor 2= limited 3= basic 4= advanced 5= expert

Proper attire includes cleats, black socks, black shorts, official referee jersey worn tucked in, 2019 badge displayed on referee jersey.

10. The referee dealt with violent conduct and serious foul play for game control and displayed the experience to
recognize when they occurred.
