Marian Greetings!

The 4th year students (79 students official enrolled) of the Information Technology
Department will be having their On-the –Job Training outside the University to expose them in real IT
profession. The students will be consuming 486 hrs in the entire OJT. Likewise may we ask for your
approval with regard to the OJT uniform that the students will be using.

In line with this may we request for the waiver for their parents sign and to be legally notarized .
