    Competition Entry form Summer 2019

    Entries need to be with money by July to League Treasurer. Please note entries on the night
    will occur a additional cost of 50p per entry in each competition. Also players need to have
    played or attended a minimum of 4 games to be eligible to enter. Entries only accepted with
    payment.
