Re: Visual Inspection of the off plot piping supports and FO Day Tank T-8111

Findings :

a. The filling and circulation line of T-703B from the header to the tank aren’t resting on the pipe support #1
and sleeper #1 on the bund wall (see pic. #1 and piping plan layout for location )
b. The circulation line of T-703AB ; the pipe shoe isn’t resting on the sleeper #2.
Location : north of H-8301 (see pic. #2 and piping plan layout for location )
c. The pipes running on the sleepers #3 & #4, east of T-703 AB ( see pic. #3 &#4).
d. P-703 Discharge Piping isn’t resting on the adjustable support (see pic. #5) .
e. FW line running on the sleeper between the MCC and 700 pump area isn’t sitting on all the sleepers (see pic.
#6)
f. MS steam header on north-south pipe rack , east of unit 100 ; the pipe shoe has somehow been twisted and
now the pipe isn’t being supported before the expansion loop .(see pic. #7)
g. T-8111 - Insulation is totally soaked with FO due to overfilling of the tank (see pic. #8).
h. T-8111 – P&V looks plugged and not working properly (see pic. #9).

The following are recommended :

a. Adjust supports , shim the sleepers with UNP or I-beams.

b. Shim the sleepers with UNP or I-beams to fill up gap of approx. 6”.
c. Shim the sleepers with UNP or I-beams to fill up gap of approx. 6”.
d. Adjust the adjustable support .
e. Fill up gap , shim the gap between EW pipe and sleepers.
f. Remove insulation and inspect what happened to the pipe and pipe shoe.
g. Renew tank insulation incl. sheeting material.
h. Urgently remove the P&V on top of T-8111, service and replace the P&V or replace
