Consent Form
1. Collection of your personal information
 In due consideration of Article 15 of the Personal Information Protection Act of the Republic of Korea, the ASEAN-
Korea Centre (hereinafter referred to as “AKC”) hereby requests your consent to collect and/or use your personal
information.
 Type of information to be collected, collection method, purpose of collection/use, and the period of retention/use
are as follows:
A. Type of Information Collected and Collection Method
- Application-related information: (English/Korean) name, date of birth, gender, nationality, email address, mobile
number, name of school, and major.
- Collection Methods: documents submission
B. Purpose of Collection/Use: Document Submission
C. Period of Retention/Use: prompt destruction 3 years after date of collection
D. You have the right to withhold consent to the collection and/or use of your personal information.
If you withhold consent, you may not proceed in the submission process.
2. Privacy and Intellectual Property
 Entrants agree that all materials produced and submitted for the Film Festival are of original work. Entrants agree
that they have previously secured the copyright of any material used in their entry. Plagiarism will not be tolerated
by the AKC, and should the AKC discover instances of plagiarism, the individual/group will be disqualified.
 The AKC reserves the right to utilize all data, videos, films, and other outputs submitted by individuals for the “2018
ASEAN-Korea Youth Short Film Competition” (hereinafter referred to as “the Film Competition”) without
limitations.
 Entrants agree that no data, results, and materials for use in the Film Festival will be disseminated by means of
publications, reports, or any other form without the express written consent from the AKC of which these products,
data, results, and materials are its exclusive property.
 The AKC reserves the right to update and change information on the Film Competition without prior notice.
3. Code of Conduct
 Entrants hereby certify that upon completion of the submission process, all information submitted is truthful and
accurate. Entrants understand that falsification of any information provided will disqualify him or her from
participation in the Film Festival, and falsification of information or impersonation will result in legal actions.
 Entrants are expected and obliged to obey the laws of the Republic of Korea while participating in the Film
Competition.
 Entrants understand with full assumption of risk and acknowledge that AKC, and their members, officers, elected
representatives and employees do not assume any responsibility for the conduct and activities of any entrant
outside of the scope of the program. If any entrant violates the regulations of the Film Competition guidelines, he
or she will be disqualified.
