    Dear Parents,
    Please read over the after school activities listed below. Students have the
    opportunity to do as many activities as they want. Kindly return the form by Friday,
    March 29th 2019 to ensure a position, students can join activities only if the signed
    form is accepted by the office. There is a minimum/maximum number of students
    per class.
    Terms and conditions are as follows:
    1. After school activities is a one term course program
    2. All fees must be settled in advance to the office
    3. A “one “ paid trial is available only on the 1st meeting of the course
    4. Fee will be refunded when cancellation notice is received before the 2nd
    meeting of the course
    5. No refund shall be permitted after the 2nd meeting of the course
    6. All “limitation of liability” apply (Section K of S.I.S admission form)
