INCIDENT OCCURRED DURING ISOLATION IN ETHLYNE TERMINAL ON 04.11.

2015

DESCRIPTION –
There was leakage in underground fire water header line which lies at north of ethylene tank
FB 2601/B at road side. Digging process was started in order to expose the line and to locate
the leaking spot. Simultaneously fire department was engaged in isolating the line. It was
decided that 8 valves need to be closed for isolating the leaking header line. For this two DV
protecting the tank FB2601/B will be remain isolated. Isolation had done but something
went wrong.

WHAT WENT WRONG


SEQUENCE-1
In afternoon a incident came into light that another DV protecting the tank FB2601/A was
found isolated and it was identified when Deficiency had reported in hydrant in compressor
area and when its spindle was repaired .Further investigation was done by fire department
and line was traced then it was found that GV-07-ETH/JEETY was in faulty condition and its
gate was dropped causing it close and was in closed condition.
SEQUENCE-2
At the same it was also rectified that gv-11-PT-ET was also in close condition and found
faulty.

Learning rom incident


(1) The Fire water network isolation valves shall be in “Lock Open or Car Seal Open” condition at all times
during normal operations.
(2) All the isolation valves, closed for isolation of network shall be suitably tagged till its restoration.
(3) The isolation valves shall have identification numbers for easy identification and maintenance of
records on extent of isolation.
(4) Inspection of valve should be done periodically.

(5) All gate valve and butterfly valve should be checked by operating.

(6) Inspection checklist of firewater isolation valve should be available.
