I hereby apply for PUAA membership to join the Alumni for advancing Punjab University’s mission and vision with a
deep sense of commitment and contributing to the best of my ability as and when needed.
Requisite Membership fee of Rs.1000 (bank draft/Demand Draft/Pay Order) has been paid in favour of Treasurer, PU
at HBL Quaid e Azam Campus Branch Lahore, A/C No. PK88HABB00018279000682301 (copy enclosed). For Online
Membership, the Membership fee has been deposited in the said account.
My particulars are given above. In case of any change, I shall initiate the same. I have read the constitution of the
Association and agree to abide by it.
