I humbly ask for apology for failing to comply the necessary requirements for our graduation which is
needed to complete the degree Bachelor of Science in Information Technology, academic year 2018 –
2019. I was not able to comply the following:

 Video Tutorial on How to use the System

 Softcopy of Capstone Project Documentation and System
 Uploaded Capstone Project System in Sourceforge.net
 Capstone Project Hardbound

In connection to this, I would like ask your consideration that you sign my graduation clearance so that I
can join this school year’s GRADUATION EXERCISES but with the following conditions:
 I will not be allowed to request my Diploma, Transcript of Records and other pertinent
documents / certification that will certify that I am graduate for the degree Bachelor of
Science in Information Technology for the school year 2018 – 2019 not unless I will
comply the lacking requirements I stated above.
 The school in response will not release my Diploma, Transcript of Records and other
pertinent documents / certification that will certify that I am graduate for the degree
Bachelor of Science in Information Technology for the school year 2018 – 2019 not
unless I will comply the lacking requirements I stated above.

I promise to comply the remaining requirements on or before March 30, 2019 - Saturday. If in any case I
cannot meet the said deadline, I hereby allow the school and its authorities to further hold all my
academic and school credentials. Thank you and God bless!
