CONTRACT OF AGREEMENT

I, ________________________, (client), in consideration of lending water dispenser, do hereby obliged to order

a minimum of __ pcs of 5 gallons of SAM PURIFIED WATER every 7 days.

Services to be Rendered/and to be noted:

a) Free lending of SAM Purified Drinking WATER dispenser;

b) No more incoming calls/waiting calls or text messages from us that can disturb in your office as the
client indicate the day of delivery for every seven consecutive days; (please let us know if you’re going
on a vacation).
c) If the clients still have loaded water gallons, clients are obliged to receive the minimum order
completely every indicated day of the week.
d) Should the client did not reach the minimum order every week, subject to fail to order within 7 days
until 8 days above, water dispenser shall be removed.

Day of delivery for every 7 days: ______________ (except Sunday. No deliveries every Sunday)

Prior to availability of water dispenser, clients are obliged to receive every indicated day of every week.
