    Sir

    I am sorry to inform you that my father Shri Sharfuddin has expired, therefore it is requested to
    kindly dissolve the our Fixed Deposit A/N. xxxxxxxxxx, Jointly by Sharfuddin and Sirajuddin
    maintained the post office of Zakir Nagar as its term is over in March 2019.

    The Principal amount and interest may be transferred to the survival of the joint account holder.

    Please attached the following document

    1. Copy of Death Certificate of Late Sharfuddin


    2. Copy of Pass Book
    3. Copy of PAN Card of Sirajuddin
    4. Copy of Employer ID (Professor , Department of Civil Engineering, Jamia Millia Islamia)
    5. Copy of Driving Licence of Sirajuddin
    6. Copy of Adhar Card of Sirajuddin
