During the second half of clinical placement,
we were able to develop a plan for our project,
implement it and evaluate it effectively. During
this process, my partner and I were able to
demonstrate professional accountability by
going to the home each week to check in with
a worker to make sure we were headed in the Choose an item.
right direction. I have also demonstrated
responsibility by communicating with my
instructor and preceptor about plans for our
project. We sent out our initial needs
assessment to many homes thanks to our
preceptor. For the development of our
presentation and powerpoint, we were able to
communicate that we would be working from
home collaboratively on the project due to the
commute distance for both of us. I
demonstrated professional responsibility by
communicating this to our preceptor.

I have demonstrated this skill by completing
our project for this placement. We were able
to take the knowledge we learned from our
initial windshield survey and from talking to
the staff members each week to guide us in the Choose an item.

We were able to take the gaps in knowledge
that we found from our initial needs
assessment and focus on that during the
development of our project. We focused on
overall digestive health instead of our original
plan to just focus on Ogilvie syndrome. We
decided to do this based on the feedback we
received from the assessment as well as the
feedback from staff in regards to claiming to
not be formally trained on digestive health
protocols.

Throughout the second half of the placement I
have continued to demonstrate this skill by
always treating the persons supported in the
home with a respectful manner every time we
show up to check in. I make sure to try to Choose an item.
engage in conversation with the staff and
clients every week to be updated on any
changes and any more ideas we can use in our
project.
I have demonstrated professional practice by
treating everyone with respect and asking
questions about our project to make sure it will
be as effective as possible.
I have also made sure to continue to maintain
confidentiality for all people in the home.
While looking at charting or making notes for
the implementation of our project, I make sure
to keep them to myself. In our project
presentation, we made sure to keep everything
general and basic so we do not disclose any
personal information.

I have worked on developing the skill of
relational practice throughout the second half
of the placement by developing a project
presentation that is specific to the population’s
needs. We were able to successfully conduct Choose an item.
an initial needs assessment, implement a
powerpoint presentation as well as an in-
person presentation to improve the overall
knowledge on digestive health and Ogilvie
syndrome.
While presenting our project, we had to be
relational in the sense to make it basic enough
for everyone to understand at any education
level. Our information on digestive health was
basic and general, but still covered important
topics related to the disabled population.
Since this was a partner project, we had to
develop the skill of collaborative practice. I
thought we were great at working together

during this placement to develop a project. As I
mentioned in the midterm, we knew each
other previously from working at the hospital
together, so it was an easy transition into
developing a project together. We were able to
work together outside of clinical time to put
together the project and have an effective
presentation as well.
Regarding the project, we were able to focus
our project on overall digestive health to meet
the needs more effectively of the staff.

Before I started this placement, I had very little
knowledge on community or public health.
After finishing this placement, along with the

placement, as well as participating in seminar

I have also been self-reflective in my nursing

practice with my goals for my career. I haven’t
changed my goal of becoming an emergency
RN, and I don’t see myself pursuing a career in
community nursing. However, I am able to take
so many new skills forward and gain a massive
appreciation for these nurses. In our short 12
weeks we were able to make a big difference
with our presentation. I can only imagine the
impact that community nurses can make
during a full time career.
