Dear Mayor Gualberto:

We, the Grade 11 Senior High School students of Don Mariano Marcos Memorial State
University, are presently enrolled in the subject Disaster Readiness and Risk Reduction. One of
its requirements is to conduct an interview and make necessary documentation of this
undertaking. We, particularly, would like to know the programs, activities and best practices of
our city regarding disaster prevention and mitigation, disaster preparedness, disaster response
and disaster rehabilitation and recovery.

In line with this, we would like to request permission to allow us interview Ms. Julie Ann B.
Hipona, the City DRRM Officer IV on April 17, 2019. Rest assured that all information we will
gather will only be used as part of our study.

Your approval to this request will surely help us to comply and complete our school
