This week I sat at the computer majority of the time trying to finish my community

resource guide for the case management department. I constructed my community resource guide

using many different resources, such as, the internet, brochures in the case management office,

community resource guide from Northeast Georgia Medical Center, and previous manuals in the

community. I had spoken with my supervisor, Tyler Williams, about whether I should create the

community resource guide including only Habersham County or Habersham County and

surrounding counties. He expressed that including the surrounding counties would be much more

beneficial for the case managers and users of the guide. I could not have agreed more with him.

Therefore, it took me at least three days to construct a 20-page resource guide that includes

everything from homeless shelters to non-emergency transportation. I tried to be very specific


with what I included by using my perspective and understanding of what previous patients have

needed or requested from the hospital during their time of discharge. I am still in the process of

making sure it includes every possible resource. The case manager, Jill, gave me a very out of

date community resource manual that they refer to at times, it was completely out of date. She

expressed that she is excited to use the one that I am creating and that they desperately need an

electronic format along with a hard copy.

Besides working on the community resource guide, I assisted the case managers with

Medicare Rights forms and calling back moderate to high risk patients. I enjoy my internship

because they always love the help that I give, and it gives me a chance to learn new skills and

material.
