Addelyn has successfully completed medication administration in a safe
manner. She utilizes clinical resources as appropriate for medication
information. She is able to provide an appropriate patient report to
the clinical faculty. Addelyn is improving in her organization and
time management by trying different "nursing brains" in order to create
an effective clinical day She could benefit from some additional

• Intervention: Meet with clinical instructor. Notify course coordinator and program
director. Academic Jeopardy Form with clear plan for resolution of concerning
behavior.
Level 1 Violation
• Unsafe practice with unacceptable risk for harm
• Examples include but are not limited to dishonesty, failure to change behaviors in
response to feedback, physical or mental impairment (intoxication or lack of sleep),
violent behavior, breach of confidentiality, extreme anxiety, angry or unprofessional
outburst.
• Intervention: Immediate removal from all clinical, lab, and simulation activities. Notify
course coordinator and program director.

Comments:
Addelyn is a joy to have in the clinical setting. She demonstrates
wonderful teamwork with the clinical team and is engaged in all aspects of
patient care. She asks appropriate questions and accepts/uses feedback to
improve her clinical skills. Addelyn will benefit from more opportunities
in the med-surg environment in order to improve her organization and
