Re: Notice of Termination of Employment

Dear ________:

We regret to inform you of the termination of your employment with Multicare


Pharmaceuticals Philippines, Inc. (MULTICARE) effective _________________. It
has regrettably been necessary to consider certain organizational and operational
changes within MULTICARE. As a result, your position as __________________ is
hereby discontinued.

While we have considered all available alternative options, however, we were compelled
to institute redundancies. The management has determined that the functions of a
______________ are considered superfluous to the operations of MULTICARE. All of its
tasks are being performed also by the President as well as the heads of every
departments of MULTICARE. Unfortunately, this means that your position will be made
redundant.

Moreover, we also believe that the creation of the position of ____________ did not
result to a positive outcome to the operations of MULTICARE. The sales of MULTICARE
dropped in the year 2017, the same period when the position of _______________ was
created.
We have attempted to identify a suitable alternative vacancy to offer you, but
unfortunately none is available. In these circumstances, we confirm that your
employment as _________________ with MULTICARE will terminate by reason of
redundancy on ______________________.

Upon termination, we will pay you the separation pay of _____________________.

If you have any queries with regard to any of the terms of this letter or your redundancy
