SYNTHESIS: This is a narrative presentation of your insights, most memorable experiences and challenges you have
experienced during your practicum journey. One page only.
RECOMMENDATIONS: After narrating your assessment during your practicum, what are then your suggestions which
you think could have been done by the School of IHTM which could’ve helped you with the challenges you have
encountered during your practicum journey. Please narrate in bulleted form.

Photographs with scholarly captions which are reflections of your actual work/duty, with your co-
trainees, supervisors/managers if allowed.
Accomplished DTR provided by the establishment and by the practicum coordinator reflecting the total
number of duty hours.

FINAL REQUIREMENTS:
PRACTICUM REPORT
There shall be NO HARD COPY of the report to be submitted. Reports will be submitted in SOFT COPY with
the following specifications/requirements:
Report Format: Letter size bond paper, Font: Arial; Font Size: 12; single space;
Practicum Report in PDF File must be sent to the following gmail account. Please strictly follow the instructions.

Subject: Establishment Family Name TSPRAC2 2SSY1819 (i.e. Lhangbiz VICTORIO TSPRAC2 2SSY1819)
DOCUMENTS:
Sealed Evaluation form, Original copy of DTR from establishment and/or SIHTM; Original Copy of Certificate of
completion (to be returned after verification); Endorsement Letter; MOA (if any)
Deadline of Submission of the soft copy of practicum report and original hard copy of the documents will be on
May 3, 2019 (FRIDAY). Final Meeting for Practicum Processing of trainings and experiences and release of
grade will be POSTED.
