HSE PLAN
1.0 Introduction / Pengenalan
The Scope of this HSE Plan is applicable during the execution of the project. The project
includes Engineering, Procurement, Supply, Construction, Transportation, installation and
pre commissioning of the Painting Project 3 x 14MW Coal Fired Power Plant Tanjung Bara.
Cakupan HSE plan ini adalah untuk diterapkan selama pelaksanaan proyek. Proyek termasuk
dalam Perencanaan, Pembelian, Pemasokan, Konstruksi, Transportasi, Installasi dan Pre-
Komissioning dari Proyek Pengecatan 3 x 18MW Coal Fired Power Plant Tanjung bara.
2.0 Health, Safety & Environmental policy Project HSE Policy
Contractor will execute the Project with foremost consideration for the Health and Safety
of personnel, protection of assets and the natural environment. Contractor will ensure
the achievement of the objectives as detailed below in support of high proje ct safety
standards: Kontraktor akan mengeksekusi proyek dengan kesadaran penuh untuk
kesehatan dan keselamatan pekerja, perlindungan aset dan lingkungan alam. Kontraktor
akan memastikan pencapaian objektive seperti disebutkan dibawah dalam mendukung
standart keselamatan project.
I. To provide a safe and healthy working environment. Untuk menyediakan

keselamatan dan kesehatan lingkungan kerja
II. To ensure Safety and health of all personnel during all work activities. Untuk
memastikan keselamatan dan kesehatan semua personel selama semua aktifitas
kerja.
III. To protect the public from injury or ill health during operations. Untuk melindungi
oranglain dari kecelakaan atau sakit selama beroperasi.
IV. To prevent loss or damage to property through proper review processes. Untuk
mencegah kerugian atau kerusakan pada properti melalui proses peninjauan
kelayakan.
V. To safeguard, protect and conserve the natural environment. Untuk melindungi dan
menjaga lingkungan alam.
VI. To comply and promote both statutory and legislative safety rules and regulations
of COMPANY. Untuk memenuhi dan mepromosikan undang-undang dan aturan
keselamatan pemerintah dan juga peraturan perusahaan.
VII. To aim to reduce the risks to the employees and assets to the lowest level. Untuk
mengurangi resiko ke karyawan dan aset menuju level terndah.
VIII. To ensure that all work procedure contains risk assessment and emergency plan.
Untuk memastikan bahwa semua prosedur kerja memuat penilaian resiko dan
rencana keadaan darurat.
IX. To ensure awareness to our employees towards health, safety and environment.
Untuk memastikan kesadaran pekerja kita dalah Kesehatan, Keselamatan dan
Lingkungan (HSE)
X. To impart appropriate training for employee's competence. Untuk memberikan
pelatihan yang layak untuk kompetensi pekerja.
Following policies signed by Director thee are attached as Appendix -1. Kebijakan berikut
ini ditandatangai oleh Direktur. Terlampir dalam Lampiran 1

6.0 Responsibilities. Tanggung Jawab
Following are the key Contractor personnel involved in the project with a brief description
of their responsibilities on HSE matters: Berikut adalah personel utama kontraktor yang
terlibat dalam projek dengan deskripsi singkat tanggung jawab mereka dalam urusan HSE :
6.1 Director / Direktur
To achieve the safety objective reflected in this plan the Project Director shall comply
and ensure compliance with all applicable Company, National and International laws and
regulations throughout the performance of the contract relating to "Health, Safety &
Environment". The following shall be the responsibilities of Project Director: Untuk
mencapai tujuan keselamatan yang terdapat dalam rencana ini, Direktur proyek harus
memenuhi dan memastikan pemenuhan hukum dan peraturan perusahaan, nasional dan
internasional yang berlaku sepanjang melaksanakan kontrak yang berhubungan dengan HSE.
Shall establish a culture of continuous improvement of "Health, Safety &
Environment' during construction activity to ensure that such matters are
considered to be of equal importance to productivity, progress, profit etc. Harus
membentuk budaya perbaikan berkelanjutan dari HSE selama kegiatan konstruksi
untuk memastikan bahwa hal ini adalah kesadaran untuk mensejajarkan sama
pentingnya dengan produktifitas, progres, keuntungan dll.
Shall demonstrate personal commitment to a zero accident performance. Harus
mendemonstrasikan komitmen dirisendiri terhadapa performa Zero Accident.
Shall ensure that the detailed requirements of the Project Health, Safety &
Environmental Plan are implemented across the project. Harus memastikan bahwa
persyaratan detai dari Project HSE Plan diterapkan diseluruh project.
Shall ensure proper selection of experienced and competent management team
members, and establishing a project Health, Safety and Environmental training plan
for all project personnel. Harus memastikan pemilihan kelayakan dari pengalaman dan
kompetensi anggota tim manajemen dan membuat Rencana Project HSE Training untuk
semua personel.
Shall set project Health, safety & Environmental performance improvement targets at
appropriate intervals throughout the project program. Harus membuat Target perbaikan
Project HSE dalam interval yang sesuai melalui project program.
Shall ensure that suitable arrangements are in place to monitor that Health, Safety &
Environmental performance targets are being achieved. Harus memastikan pengaturan
yang cocok dalam memonitor target performa HSE yang akan dicapai.
Shall establish a direct line of communication with the key personnel of the project on
Health, Safety & Environment matters. Harus membuat komunikasi langsung dengan
personel utama dari project dalam hal HSE
Shall establish a monitoring regime to ensure that unsafe systems, unsafe act are
identified and remedied at the earliest stage. Harus membuat monitoring untuk
memastikan sistem tidak aman, tindakan tidak aman diindentifikasi dan diperbaiki
dalam tahap awal.
Shall provide all members of his project management/supervisory team with
adequate direction, support and encouragement to fulfill their respective Health,
Safety and Environmental duties and responsibilities. Harus menyediakan semua
anggota dari tim supervisor / manajemen proyek dengan arahan yang memadai,
dukungan dan dorongan tuntuk memenuhi masing-masing kewajiban dan
tanggungjawab HSE.
Shall ensure that the attendance at Health, safety and Environment meetings is
comprehensive and appropriate. He will also ensure that the minutes of such

meetings assign actions and timings to named individuals and are promptly
distributed to all relevant personnel. Untuk memastikan bahwa kehadiran meeting HSE
komprehensif dan sesuai. Juga memastikan bahwa hasil dari meeting menetapkan
langkah dan waktu dan nama individu dan segera mendistribusikan ke semua presonel
yang sesuai.
 Shall ensure that COMPANY / Local regulations pertaining to Health, Safety &
Environment norms are complied with through out the project activities. Harus
memastikan Peraturan Perusahaan / lokal berkaitan dengan Norma HSE sudah
dipenuhi dengan aktifitas project.
6.2 Project Manager / Project manager

• Shall set the project HSE policy, objectives, and targets and shall monitor their
implementation as per the contractual requirements. Harus membuat Kebijakan HSE
Project, OTP dan memonitor penerapannya sesuai dengan persyaratan kontrak.
• Shall review / amend and update HSE Plan at least once in a year; however
amendment to this document shall also be done from time to time depending on
deviations and observations of Contractor and Company. Harus meninjau /
mengamandemn dan mengupdate HSE plan kurang lebih setahun sekali, namun
merevisi dokumen ini juga harus dilakukan setiap saat tergantung dari perbedaan dan
observasi dari kontraktor dan perusahan.
• Shall allocate resources (financial, material and personnel) for the attainment of HSE
objectives and targets. Harus mengalokasikan sumberdaya (keuangan, material dan
personel) untuk pencapaian HSE OTP.
• To assume overall responsibility on HSE matters with regard to the conformance to
the overall project safety system. Menganggap semua tanggung jawab dalam hal
HSE dengan anggapan kesesuaian dari semua sistem keselamatan project.
• Shall be the Chairman of the HSE committee. Harus menjadi Chairman dari komite
HSE
• Shall review and approve training programs at site in consultation with the Safety
Manager. Harus meninjau dan mensetujui program pelatihan dilapangan dalam
konsultasi dengan manjer keselamatan
• To demonstrate, by his personal experience, his commitment and personal concerns
for HSE related matters. Untuk mendemonstrasikan, dengan pengalaman pribadi,
komitmen dan perhatian pribadi untuk masalah yang berhubungan dengan HSE.
• Shall ensure that all accidents/incidents, occupational sickness and environmental
incidents are thoroughly investigated and effective measures are taken to prevent
recurrence. Harus memastikan bahwa kecelakaan / cidera, penyakit akibat kerja dan
pencemaran lingkungan adalah diinvesitigasi secara terus menerus dan engukuran
dilakukan untuk mencegah terjadi lagi.
• Shall be responsible for conformance to the Project Safety Systems and procedures
of all construction personnel, including subcontractor under his Authority. Harus
bertanggungjawab dalam ksesesuaian sistem keselamatan project dan prosedur dari
semua personel konstruksi termasuk subkontraktor dibawah naungannya.
• Shall have thorough knowledge of Company's Safety requirements. Harus memiliki
pengetahuan menyeluruh dari persyaratan keselamatan Perusahaan.
• Shall be responsible for ensuring all plant and equipment used for Project work are
regularly inspected, properly certified, maintained and operated in conformance with
the manufacturer's recommendations. Harus bertanggungjawab dalam memastikan
semua plant dan peralatan yang digunakan dalam perkerjaan di ispeksi secara rutin,
disertifikasi, dirawat dan diopersikan sesuai dengan rekomendasi pabrikan.
• Shall ensure that all personnel under his authority have the relevant competency and
experience to execute their work safely. Harus memastikan semua personel dibawah
naungannya memiliki kompetensi dan pengalaman yang seuai untuk melakukan
pekerjaannya dengan aman.
• Shall ensure work procedures including COMPANY's HSE requirements and hazard

identification, are understood by supervisors/workers. Harus memastikan prosedur

kerja termasuk Persyaratan HSE Perusahan atau identifikasi bahaya dimengerti oleh
supervisor / pekerja.
• Shall conduct work site inspection and promote safety among workers to ensure a
high level of Safety awareness. Harus melakukan inspeksi area kerja dan
mempromsikan keselamatan diantara pekerja untuk memastikan kesadaran
keselamatan dalam level tinggi.
• Shall ensure machinery and equipment are inspected by competent person(s) and
such inspection findings are documented and properly reviewed. Harus memastikan
mesin dan peralatan diinspeksi oleh orang yang kompeten dan temuan inspeksi
didokumentasikan dan ditinjau dengan baik.
• Shall ensure site toolbox meetings are held regularly. Harus memastikan toolbox
meeting dilakukan secara rutin.
• Shall support and encourage his supervisors to maintain safe working practices in all
the activities. Harus mendukung dan mendorong supervisor untuk menjaga praktek kerja
aman disemua kegiatan.
• Shall manage all subcontractors to ensure that overall Safety compliance is
achieved. Harus mengatur semua subcontractor untuk memastikan bahwa semua
persyaratan keselamatan sudah terpenuhi.
• Shall enforce appropriate disciplinary action towards any breach of the Project Rules
and Procedures. Harus menindakpelanggaran dai peraturan project dan prosedur
dengan hukuman yang sesuai.
6.3 HSE Manager (HO) / Manajer HSE

• Shall report to Project Director. Harus melapor ke Direktur Project
• Shall review safety systems and work procedures to ensure that it confirms
with COMPANY safety system. Harus meninjau sistem keselamatan dan prosedur kerja
untuk memastikan bahwa itu sesuai dengan Sistem Keselamatan Perusahaan.
• Shall enforce appropriate disciplinary action towards any breaches of the Project
Rules and Procedures. Harus menindakpelanggaran dai peraturan project dan
prosedur dengan hukuman yang sesuai
• Shall monitor the implementation of the HSE plan and appraise the Project
Director on all Safety matters related to the project. Harus memonitor penerapan
HSE plan dan memberitahukan project direktur dalam hal keselamatan terkait dengan
project.
• Shall be a member of risk assessment team and areas requiring more attention,
risk reduction or systems improvement. Harus menjadi anggota tim penilaian resiko
dan area yang memerlukan perhatian lebih, pengurangan resiko atau perbaikan sistem.
• Shall act as the Safety auditor and shall liaise with Company. Harus bertindak
sebagai Auditor keselamatan dan bekerjasama dengan perusahaan.
• Shall set project Safety targets and plans in consultations with Project Director to
achieve the Safety performance targets of the whole project. Harus membuat target
keslamatan project dan rencana dalam konsultasi dengan Project Direktur untuk
mencapai Target performa keselamatan dari keseluruhan project.
• Shall visit site and conduct safety audits along with Construction Manager and
Safety Coordinator and HSE training programme on monthly basis. Harus mendatangi
lokasi dan melakukan audit keselatan yang berhubungan ddengan manager konstruksi
dan Safety.
6.4 Site Manager / Site Manager

 Shall report to Project Manager. Harus melapor ke project manager
 Shall be responsible for conformance to the Project Safety Systems and procedures
of all construction personnel, including subcontractors under his Authority. Harus
bertanggungjawab untuk kesesuaian dengan sistem keselamatan projec dan prosedur

dari semua personel konstuksi, termasuk subkontraktor di bawah naungannya.

Shall comply all local and COMPANY Health, Safety & Environment requirement
before commencement of activity. Harus memenuhi semua persyaratan HSE
perusahaan / lokal sebelum melakukan pekerjaan.
Shall be responsible to ensure that all plant and equipment used for Project work are
regularly inspected, certified by competent persons / agency, maintained
and operated in conformance with the manufacturer's recommendations. Harus
bertanggungjawab dalam memastikan semua peralatan yang digunakan untuk
pekerjaan diinspeksi secara rutin, disertifikasi oleh orang yang berkompeten / agensi,
dirawat dan dioperasikan sesuai dengan rekomendasi pabrikan.
Shall ensure that all personnel under his authority have the relevant competency and
experience to execute their work safely. Harus memastikan bahwa semua personel di
bawah naungannya memiliki kompetensi dan pengalaman sesuai untuk melakukan
pekerjaannya dengan aman.
Shall ensure work procedures including COMPANY, HSE requirements and hazard
identification, are understood by supervisors / workers. Harus memastikan prosedur
kerja termasuk perusahaan, persyaratan HSE dan Identifikasi bahaya dimengerti oleh
supervisor / pekerja.
Shall ensure that work procedures contain Safety requirements and hazards are
identified, assessed, properly managed and are understood by supervisors /
workers. Harus memastikan bahwa prosedur berisi persyaratan keselamatan dan bahaya dinilai
dan diidentifikasi, dikelola dengan baik dan dimengerti oleh supervisor dan pekerja.
Shall conduct work site inspection and promote safety among workers to ensure a
high level of Safety awareness. Harus melakukan inspeksi lokasi kerja dan
mempromosikan keselamatan diantara pekera utuk memastikan kesadaran keselamatan
dalam level tinggi,
Shall carry out safety walkabout session and safety audit regularly with all work
sections engaged in the project activity. Harus melakukan sesi Safey Walkabout dan
Audit keselamatan secara reguler dengan semua seksi pekerjaan yang terikat dalam
aktifitas proyek.
Shall ensure site toolbox meetings are held before commencement of work and all
HSE requirements are communicated to work crew engaged in the activi ty. Harus
memastikan toolbox meeting dilakukan sebelum memulai pekerjaan dan semua
Persyaratan HSE telah disampaikan ke pekerja yang terlibat dalam pekerjaan tersbut.
Shall support and encourage his supervisors to maintain safe working practices.
Harus mendukung dan mendorong supervisor untuk menjaga praktek kerja aman.
Shall ensure all subcontractors engaged in the project activity are complying Health,
Safety & Environmental requirements. Harus memastikan semua subkontraktor terikat
dalam kegiatan project memenuhi Persyaratan HSE.
Shall enforce appropriate disciplinary action towards any breach of Project Health,
Safety and Environmental Rules and Procedures. Semua pelanggaran dari prosedur
HSE harus ditindak dengan hukuman yang sesuai.
Shall be responsible for ensuring all plant and equipment used in construction
activity are regularly inspected, maintained and are in a safe working condition. Harus
bertanggungjawab dalam memastikan semua peralatan yang digunakan dalam
kegiatan konstruksi sudah diinspeksi secara rutin, dirawat dan dalam kondisi kerja
aman.
Shall attend weekly safety meeting and ensures construction site safety concerns
are addressed. Harus menghadiri meeting Safety mingguan dan memastikan
masalah keselamatan lokasi konstruksi ditangani.
Shall work in close co-operation with the HSE personnel to eliminate and correct all
practices and conditions that are deemed to be unsafe. Harus bekerja sama dengan
HSE personel untuk mengeliminasi dan membetulkan semua kegiatan dan kondisi
yang dianggap tidak aman.

 Shall comply all statutory requirements applicable for the project. Harus mematuhi
dengan semua persyaratan undang-undang yang sesuai dengan project.
 Shall report all incidents / injuries to HSE Site Coordinator and take immediate action
to secure the scene and prevent recurrence. Harus melaporkan semua kecelakaan dan
cidera ke koardinator HSE dan mengambil tindakan aman dan mencegah terulang lagi
6.5 Site safety Coordinator / Safety Koordinator

 Shall organize meetings and presentations to communicate Health, Safety &
Environmental legislative requirements of Republic of Indonesia and Company. Harus
membuat meeting dan persentasi untuk mengkomunikasikan peraturan HSE dari
pemerintah / perusahaan.
 Shall organize, co-ordinate for planned Safety training and awareness programs as
per the need of construction activity. Harus membuat, mengkoordiasikan utuk rencana
pelatihan safety dan program kesadaran sesuai dengan kebutuhan kegiatan konstruki.
 Shall monitor the implementation of the HSE plan and appraise the Project Manager
on all Safety matters related to the project. Harus memantau penerapan HSE plan dan
memberitahukan projecy manager dalam semua hal terkait HSE project.
 Shall organize and attend HSE committee meetings. Harus membuat dan menghadiri
HSE Committe meetings
 Shall coordinate internal and Company Health, Safety & Environmental
inspection/safety audit. Harus berkoordinasi Audit / Inspeksi HSE internal dan
perusahaan.
 Shall review project Safety targets and plans in consultations with Project manager
to achieve the Safety performance targets. Harus meninjau target keselamatan dan
merencanakan dalam konsultasi dengan project manager untuk mencapai target
performa keselamatan.
 Shall enforce appropriate disciplinary action towards any breach of the Project
Rules and Procedures. Semua pelanggaran dari prosedur HSE harus ditindak dengan
hukuman yang sesuai.
 Shall liaise with Company representative and other similar competent persons
and shall take account of advice rendered. Harus bekerjasama dengan perwakilan
perusahaan dan orang yang kompeten lainnya dan harus mempetimbangkan saran
yang diberikan.
 Shall ensure that procedures contained in this plan are revised regularly as dictated
by situation. Harus memastikan prosedur yang terkandung dalam rencana ini ditinjau
secara rutin sebagaimana ditentukan oleh situasi.
 Shall ensure that accident/ incident are reported, investigated for root cause and
effective measures are taken and monitored regularly. Harus memastikan bahwa
kecelakaan / cidera dilaporkan, diinvesitgasi akar permasalahannua dan diukur dan
dimonitor secara rutin.
 Shall monitor the project management systems and work permit systems to ensure
that the arrangements established adequately address, Health, Safety and
Environment. Harus memantau sistem project managemen dan sistem ijinkerja untuk
memastikan bahwa pengaturan HSE sudah cukup ditangani.
 Shall conduct safety induction training of all employees engaged in the
activity/project. Harus melakukan training induksi keselamatan semua pekerja yang terlibat

 Shall conduct daily visit of construction site and shall advise/assist supervisors to

6.6 Section Managers (Plant and equipment, Quality Control and Others)
Section Managers shall be responsible for: Seksi Manager harus bertanggungjawab untuk ;
 Demonstrating their personal commitment to and participating in initiatives designed

to continuously improve Health, Safety and Environmental performance and achieve
a zero accident performance. Menunjukan komitmen personal untuk dan
berpartisipasi secara insisiatif dalam perbaikan HSE performance secara
berkelanjutan dan mencapai performa Zero Accident.
 Compliance of all legislation of Republic of Indonesia and Company requirement in
their area of control. Mematuhi semua peraturan Pemerintah dan Perusahaan
 Implementing the detailed requirements of Health, Safety and Environment policy
and the Project Health, Safety & Environmental plan. Mnerapkan persyaratan detai dari
HSE policy dan HSE Plan
 Organizing the management supervisory structure within their respective sections to
ensure that the detailed requirements of the Health, Safety & Environment is
understood and implemented by all personnel engaged in their respective sections.
Menyusun struktur management pengawas dengan masing-masing seksi untuk
memastikan bahwa persyaratan HSE telah dimengeri dan dilakukan oleh personel
yang terikan dengan seksi masing-masing.
 Ensuring that personnel under their control are suitable and competent to adequately
discharge their respective duties and responsibilities. Memastikan semua personel
dibawahnya sesuai dan kompeten untuk memenuhi kewajiban dan tanggungjawab
masing-masing.
 Continuously reviewing health and safety performance within their sections to ensure
that performance targets are being achieved. Secara terusmenerus meninjau performa
HSE dengan seksi mereka untuk memastikan target telah dicapai.
 Ensuring that the specific risk assessments conducted by trained, competent
personnel at the appropriate time, and that the resultant control measures are
communicated to the persons responsible for supervising and executing the work.
Memastikan bahwa penilaian resiko spesifik telah dilakukan oleh orang terlatih,
kompeten dalam waktu yang sesuai dan langkah-langkah pengendalian yang
dihasilkan telah dikomunikasikan ke orang yang bertanggungjawab untuk
pengawasan dan pelaksanaan pekerjaan.
 Ensuring that all management and supervisory personnel engaged within their
section conduct routine Health Safety & Environmental surveillance as they go about
their day-to-day work. Memastikan bahwa semua managemen dan personel pengawas
terikat dengan pelaksanaan pengawasan HSE seksi mereka seperti keseharian bekerja.
 Ensure that plant and equipment used in their area of control are maintained,
inspected and certified by competent agency as per rule of Republic of Indonesia
and as per COMPANY requirements. Memastikan peralatan yang idgunakan di area
dikontrol, dirawat, diinspeksi dan disertifikasi oelh agensi yang berkompeten sesuai
dengan persyaratan pemerintah dan perusahaan.
 Shall ensure that all crew in their area of control have received safety induction
program and training program required to enhance HSE performance. Harus
memastikan bahwa semua crew di dalam area kontrol telah mengikuti program safety

6.7 HSE Officer

 Shall report to HSE Coordinator at site. Harus melapor ke HSE Koordinator
dilapangan
 Shall monitor safety systems, and emergency services at work site. Harus memantau
sistem keselamatan dan servis keadaan darurat di tempat kerja.
 Shall conduct Safety induction training for all employees engaged in the activity /
project. Harus melakukan safety training induksi untuk semua pekerja yang terlibat dalam
kegiatan / pekerjaan
 Shall conduct daily visits of construction sites and shall advise / assist supervisors to
maintain good housekeeping, safety practices and emergency services. Harus
melakukan kunjungan harian di lapangan dan harus menasehati / mengawal
supervisor untuk menjaga houskeeping yang baik, praktek keselamatan dan servis
keadaan darurat.
 Shall regularly inspect emergency services and give report to concerned departmet
for compliance of deficiency and for provision of trained and qualified personnel to
maintain the emergency services. Harus secara rutin inspeksi servis keadaan darurat
dan melaporkan ke departement terkait untuk kesesuaian dari perbedaan dan
ketersediaan personel terlatih dan qualify untuk merawat servis keadaan darurat.

 Shall assist HSE coordinator/ investigation group for incident investigation. Harus
memandu HSE koordinator / investigasi grup saat invstigasi kecelakaan.
 Shall Co-ordinates Company/ CONTRACTOR Safety inspection/ walkabout
Sessions. Harus memandu Safety Inspeksi / sesi walkabout Perusahaan / kontraktor.
 Carries out daily tour of construction sites with supervisors to coordinate and
maintain a good house keeping and safe practices. Melakukan tour harian di area
konstruksi dengan supervisor untuk koordinasi dan menjaga houskeeping yang baik
dan praktek kerja aman.
 Shall check and ensure that precaution reflected in work permits are complied with.
Harus cek dan memastikan bahwa tindakan pencegahada di ijin kerja dan sesuai

Shall check and ensure that all work permit (main work permit and complimentary
permits), enclosure such as list of equipment, approved Procedures, risk
assessment, schematic layout of the working area are attached and duly fille d before
submission of work permit. Harus cek dan memastikan bahwa semua ijin kerja (ijin kerja
utama dan tambahan), dilampirkan seperti daftar peralatan, prosedur yang disetujui,
Penilaian resiko, denah area kerja dan sepatutnya diisi sebelum mengajukan Ijin kerja.
Shall check validity of work permit and apply for extension / new fresh permit as and
when required in consultation with planning department. Harus cek validitas ijin kerja
dan mengajukan untuk perpanjangan / permit baru dan jika perlu konsultasikan dengan
departement planing.
Shall retain one copy of work permit and record it and shall give one copy to planning
department for records. Harus menahan satu salinan ijin kerja dan menyimpannya dan
harus memberikan satu salinan ke departemen planing untuk disimpan
Shall monitor that a copy of the work permit(s) is displayed at the worksite at all
times. Harus memantau bahwa salinan ijin kerja ditampilkan di area kerja setiap waktu.
Shall maintain work permit register and reflect the status of work permit regularly in
work permit register. Harus menjaga register ijin kerja dan menampilkan status ijin kerja

Shall inform reflected precautions in work permit to planning, execution, maintenance
and work supervisors for compliance before handing over the work permit to
supervisor. Harus menginformasikan tindakan pencegahan diijin kerja untuk
perencanaan, eksekusi, perawatan dan kerja supervisor untuk pemenuhan sebelum
penyerahan ijin kerja ke supervisor.
Shall monitor that supervisor in charge of the overall work activity is well aware of all
the tasks associated with the particular jobs. Harus memonitor bahwa supervisor yang
bertugas telah paham semua kegiatan bekerja dari semua tugas yang berhubungan
dengan pekerjaan sejenis.
Shall stop the work if operational condition has been changed and creating
hazardous situation or the persons executing the work are not observing the
precautions. Harus memberhentikan pekerjaan jika kondisi operasional telah berubah
dan menimbulkan situasi yang berbahaya atau orang yang bekerja tidak melakukan
obserbasi tindakan pencegahan.
Shall regularly close out the work permit after a specified period or after completion
of work. Harus menutup secara rutin ijin kerja setelah periode sesesai atau setelah pekerjaan
selesai
Shall comply with the work permit procedure. Harus memenuhi prosedur ijin kerja.
6.8 Construction Supervisor

 Shall report to the Site work Manager. Harus melaporkan ke site manager
 Shall ensure all workers are aware of emergency procedures. Harus memastikan
semua pekerja tau dengan prosedur keadaan darurat.

Shall give safety as a first priority during his daily work supervision. Harus paham
bahwa safety adalah prioritas pertama selama mengawasi pekerjaan harian.
Shall conduct Toolbox talks with reference to Risk assessment and hazard to the
personel enggaged for the related job to ensure his working crews are made aware
of hazards that are to be encounterd during executing the job. Harus melakukan
toolbox talk dengan refrensi Penilaian resiko dan mengkomunikasikan bahaya ke
personel yang terlibat untuk memastikan anggota waspada terhadap bahaya yang
timbul selama melakukan pekerjaan.
Shall adhere to hazard management controls. Harus mengikuti manajemen control
bahaya.
Shall ensure that all the members of the working crews have gone through the safety
induction training and have been issued the required Personal Protective Equipment.
Harus memastikan bahwa setiap anggota yang bekerja telah mengikuti Induksi dan
telah memiliki PPE yang disyaratkan.
Shall monitor and enforce for wearing of PPE by his working crew. Harus memantau
dan melaksanakan penggunaan APD oleh anggota
Shall organize weekly Safety committee meeting and submit the report to
CONTRACTOR representative and HSE manager. Harus menyusun meeting
minggan safety committee dan melaporkan ke perwakilan kontraktor dan HSE
manager.
Shall carry out weekly inspection checks of all the tools and equipment to ensure that
they are in good working condition. Harus melakukan inspeksi mingguan ke semua
peralatan untuk memastikan dalam keadaan baik untuk bekerja.
Shall maintain good housekeeping at site as per the documented project standard.
Harus menjaga Housekeeping yang baik ditempat kerja sesuai dengan standard
prosedur.
Shall checks that employees are trained in the proper use of tools/machinery
/equipment. Harus cek bahwa pekerja telah dilatih dalam penggunaan alat/mesin dengan
baik.
Shall organize weekly emergency evacuation drill and give report to
CONTRACTOR representative and HSE manager. Harus menyusun drill evakuasi
keadaan darurat mingguan dan melaporkannya ke perwakilan kontraktor dan HSE
manager.
Shall ensure that work locations are equipped with safety requirement such
as Fire Extanguiser, Emergency Kit, etc. Harus memastikan bahwa lokasi kerja
dilengkapi dengan peralatan keselamatan misalnya APAR, P3K
Shall ensure prior arrangements are made for emergency evacuation and shall
check emergency preparedness items viz., first aid arrangement, transportation
arraignment, and emergency plan. Harus memastikan pengaturan sebelumnya telah
dibuat untuk evakuasi keadaan darurat dan harus cek item kesiapan keadaan darurat
seperti, P#K, Alat transportasi, dan rencana keadaan darurat.
Shall ensure that, in daily toolbox meeting, points of emergency response plan are
communicated. Harus memastikan bahwa dalam toolbox meeting harian poin dari
rencana keadaan darurat disampaikan.
6.9 All Personnel / Semua Personel
 All personnel shall have attended Safety induction briefing before commencing
work on site. Semua personal harus menghadiri safety induction sebelum melakukan
pekerjaan di lokasi.
 All personnel shall conform to the Company standard of Personnel Protective
Equipment whenever present on the work site. Semua personel harus sesuai dengan

standard PAD perusahaan ketika ada di area kerja.

 All personnel shall take all necessary precautions to ensure that their activities do not
affect adversely to other working personnel involved in other activities. Semua
personel harus mengambil tindakan pencegahan yang diperlukan untuk memastikan
kegiatan mereka tidak berdampak buruk untuk personel lain yang bekerja dipekerjaan
lain.
 All personnel shall be assessed by HSE Co-ordinator for their competency to
perform the job. Their HSE rating shall be done as per Company norms. Semua
personel harus dinilai oleh HSE coordinator untuk kompetensi dalam bekerja. HSE
rating pekerja harus dibuat sesuai dengan norma Perusahaan.
 All personnel shall ensure compliance of safety standards reflected in HSE norms.
Semua pekerja harus memastikan pemenuhan standard keselamatan yang ada di
peraturan HSE.
 All personnel shall work according to the instructions of their supervisors and always
comply with the Safety plan, Safety procedures, and general safe work practices.
Semua pekerja harus bekerja bedasarkan instruksi dari supervisor mereka dan selalu
sesuai dengan Safety plan, Safety prosedur dan praktek umum bekerja aman.
 All personnel shall maintain tools and equipment, issued to them in a safe operating
condition and shall report any defects observed to their immediate supervisor without
delay. Semua personel harus menjaga peralataan, diberikan alat dalam keadaan aman dan
harus melaporkan semua kerusakan sesegera mungkin ke supervisor.
 All personnel shall act as per instruction of Supervisor in the event of emergency.
Semua personel harus bertindak sesuai dengan instruksi suprevisor dalam situasi
keadaan darurat.

the project. Kontraktor telah menerapkan ZERO Kecelakaan sebagai tujuan HSE selama
eksekusi project.

9.0 Safety Orientation Programme / Program Orientasi keselamatan
Any new employee joining the project shall be sent to the HSE department by
Administration department for Safety induction. On completing attendance at the
Contractor's in-house Safety induction, each employee shall acknowledge safety
induction format to confirm that he has understood the contents and shall abide by the
safety rules to comply with safe work practices. Personnel identity card to the employee
shall be issued only after having completed the safety induction training, which will
provide him legal access to the work site. A register shall be maintained containing list
of all personnel having attended the Safety induction training. Karyawan baru yang
bergabung di proyek harus mengikuti safety induction oleh HSE Department. Dalam
pemenuhan kehadiran di Kontraktor Inhouse safety Induction, setiap pekerja harus
menjawab safety induction format untuk mengkonfirmasi bahwa dai telah mengerti isi dan
harus mematuhi aturan keselamatan untuk memenuhi praktek bekerja aman. Kartu
identitas personel harus diterbitkan hanya setelah mengikuti safety induction, yang akan
memberikan akses legal ke lokasi kerja. Daftar hadir harus ada yang berisi daftar dari
personel yang mengikuti safety induction.
10.0 Training / Pelatihan
To achieve our safety objectives and to meet the Zero Accident Targets, safety training
shall be regularly organized for all personnel working on the Project. All managerial and
supervisory staff shall attend Safety Training Courses, organized by COMPANY.

people aware of the hazards & dangers to prevent environmental pollution on site:

10.1 Emergency Response Training Drills and exercises / Pelatihan drill Keadaan
darurat.
Regular emergency drills and exercises shall be carried out to test preparedness of
emergency arrangements. Drills and exercise shall be conducted at night also.
Emergency drill dan pelatihannya harus dilakukan untuk tes kesiapan pengaturan darurat.
Drill dan pelatihan harus dilakykan dimalam hari juga.
All personnel engaged in particular project site shall participate in emergency response
drill. Emergency evacuation drill shall be organized on likely incidental scenario.
Emergency evacuation drill register shall be maintained containing information for type
of drill, location of drill, participants and observations. Before organizing mockup drill, the
information shall be sent to COMPANY representatives to observe deficiencies, if found
during drill and suggestions for improvement. Semua personel yang terikat dalam
project harus berpartisipasi dalam drill keadaan darurat. Evakuasi drill harus disusun
seperti sekenario mendadak. Drill evakuasi keadaan darurat harus mengandung
informasi berupa tipe drill, lokasi drill, partisipasi dan observasi. Sebelum menyusun
mockup drill, informasi harus disampaikan ke perwakilan perusahaan untuk observasi
kekurangan, jika ditemukan selama drill dan disarankan untuk perbaikan.

10.2 Training on handling of equipments / Pelatihan menangani peralatan.
Training shall be imparted to the crewmembers engaged for handling and maintenance
of equipment by the Maintenance Manager. Training register shall be maintained
containing information of date, name of participant and type of training imparted. Pelatihan
harus disampaikan ke anggota terkai untuk menangani dan ,erawat peralatan oleh manager
maintenance. Daftar hadir pelatihan harus diisi informasi tanggal, nama dan pelatihan yang
disampaikan.
11.0 Meetings - Safety Communication & Consultation Rapat – Komunikasi Safety dan
Konsultasi
All personnel engaged at project shall attend HSE meetings and safety talks. HSE
department shall organize regular specific HSE meetings among senior staff of the
project. The HSE meeting shall be minuted and copies of minutes are forwarded to all
concerned for action and implementation. Semua personel yang terlibat dalam project

Internal safety committee meeting shall be organized monthly to monitor effectiveness of

employees to whom they have found suitable for giving a safety award on his efforts for

safety meetings organized by COMPANY. The COMPANY shall fix the agenda of the
meeting. The contents of the previous meeting shall also be reviewed and discussed.

Before starting new activities or activities which are more hazardous or dangerous and
require special safety precautions, Job specific Toolbox meeting shall be organized by
work supervisor to discuss safe methods of work. The Subject matters to be discussed
with working crew shall highlight specifically the hazards anticipated during execution of

All incident and accidents shall be recorded and reported within 12 hours. Accidents
shall include any occurrence, which caused injury (even if no lost time involved), harm to
the environment, and damage to equipment or loss of production. All incident and
accident shall he notified to COMPANY immediately. The investigation team shall be
formed and the full report of the incident/accident shall be communicated in writing to the

Recommendations from the accident/incident investigation shall be implemented and

monitored for effectiveness. An incident register shall be maintained and forwarded to

harus dilakukan dan dipantau efektifitasnya. Sebuah daftar kecelakaan harus
dipelihara dan diteruskan ke perusahaan jika diperlukan.

12.1 Man hours and Incident statistics Manhours dan Statistik Insiden
The Safety statistics shall be tabulated on monthly basis and sent to COMPANY. Details
of the following classification of statistics related to injuries, non-injuries and near miss
incidents shall be tabulated. Sadety statistik harus ditabulasikan dalam basis bulanan dan
dikirim ke Perushaan. Detail klasifikasi berikut dari statistik berhubungan dengan cidera,
non cidera dan near miss insiden harus ditabulasikan.
Standard format, consisting of the following classified headings shall be tabulated: Format

12.2 Rehabilitation / Rehabilitasi
Contractor shall be responsible for the effective rehabilitation of any of its employees
who are injured while working on the project. The objective is to assist in the early, safe
return of an employee to meaningful and productive work following an injury and to
integrate the injured employee successfully back into the workforce where possible. This
shall be done to ensure the following but not limited to: Kontraktor akan bertanggung jawab

Persons who continually ignore safety rules or procedures or through their deliberate
actions, which result in health and safety of themselves and others at risk, shall be

It shall be responsibility of the Maintenance Manager to acceptance certificate.

For equipment, which requires third certification system shall not be deemed to
supersede such Onshore vehicles shall be equipped with Safety seat belt and licensed

Operators who have to operate crane shall be adequately trained and conversant with

Any new crane operator on a work site shall be required to go through a test to confirm

Crane shall be certified by an independent surveyor at least every 3 years or as per

applicable norms of the project. All slings wire ropes, chain and shackle shall be
inspected annually and marked with the appropriate identification number, SWL and color-
code. Crane harus disertifikasi oleh surveyor independen setidaknya setiap 3 tahun atau sesuai
norma yang berlaku dari proyek. Semua tali sling kawat, rantai dan belenggu harus diperiksa

All lifting tackles shall be inspected and certified as per the project requirement. Only
certified lifting tackles shall be used for lifting purposes. Semua penanganan pengangkatan
harus diperiksa dan disertifikasi sesuai dengan persyaratan proyek. Hanya pengangkatan
pengangkatan yang disertifikasi harus digunakan untuk tujuan pengangkatan.
All lifting tackle shall have color-coding, which shall be changed on intervals as per
guidelines laid down by COMPANY. A rigging register shall be maintained for D-
shackles, B-shackles, Grab hooks, wire rope slings etc. The content of rigging register

13.4 Daily workplace inspections / Inspeksi harian tempat kerja

To identify potential hazard and to initiate corrective actions, each work supervisor shall
conduct daily inspection. Contractors Representative, barge Superintendent and site
safety supervisor shall supplement this. Untuk mengidentifikasi potensi bahaya dan untuk

shall conduct weekly inspection along with barge superintendent and construct ion

All electrical appliances used at project site shall be inspected fortnightly and tagged. An
electrical appliances register shall be maintained to control the inspection periodi city.
Defective electrical appliances shall not be used for construction purpose until rectified.
The content of electrical appliance register shall contain serial no of machine, type of
equipment, condition, date of inspection, next due date and remark. Semua peralatan listrik

14.0 Safety audits, Walkabout / Joint safety Inspection / Audit Safety, Walkabout / Inspeksi

A program of planned and documented Safety Audits / Review will be carried out in
order to provide objective evidence of the compliance to Safety requirements and
procedures. Inspection team comprising of CONTRACTOR Representative, by.
CONTRACTOR Representative and Work supervisors shall conduct management
Walkabout. Observations made during walkabout session shall be recorded. Program
Audit / Tinjauan Keselamatan / Audit yang direncanakan dan didokumentasikan akan
dilakukan untuk memberikan bukti objektif tentang kepatuhan terhadap persyaratan dan

Audit shall be carried out in order to: Audit harus dilakukan untuk:
Provide objective evidence of conformance with agreed systems, methods and

15.0 Risk assessment / Penilaian Risiko
The risk assessment is a tool, which provides a list of planned work activities and
identifies safety activities required for the hazard control and allocates responsibilities for
implementing controls. Before starting any activity risk assessment shall be done and
forwarded to COMPANY for approval. Penilaian risiko adalah alat, yang menyediakan daftar
kegiatan kerja yang direncanakan dan mengidentifikasi kegiatan keselamatan yang diperlukan

Once the risks have been identified then the hazards can be dealt with using the
recommended methods. A risk assessment is an essential requirement for issuing work
permits to commence the work. No activity such as welding, lifting of
spool, diving, and transportation of personnel, NDT, transportation of material shall be
started unless COMPANY approves it. Risk assessment shall be used during pre-
mobilization and pre-job reviews involving the relevant Supervisors and work groups.

15.1 Hazard Assessment / Penilaian Bahaya

Project management, safety personnel and supervisors shall at all times encourage
personnel to identify and correct hazards as a part of their normal duties. Hazard report
forms shall be made available to all operational personnel. If an identified hazard cannot
be rectified immediately, the area affected by the hazard is to be barricaded and warning
signs shall be displayed. Supervisor shall immediately follow up for the rectification of
the hazard. Manajemen proyek, personel keselamatan dan pengawas harus selalu
mendorong personel untuk mengidentifikasi dan memperbaiki bahaya sebagai bagian
dari tugas normal mereka. Formulir laporan bahaya harus tersedia untuk semua personil
operasional. Jika bahaya yang teridentifikasi tidak dapat diperbaiki dengan segera, area
yang terpengaruh oleh bahaya harus dibarikade dan tanda-tanda peringatan harus
ditampilkan. Pengawas segera menindaklanjuti perbaikan bahaya tersebut.
Safety Supervisors shall conduct daily / inspections at site and identify such potential
hazards and take immediate corrective measures. Throughout the duration of the

project, hazard identification techniques shall be utilized. Hazards and risks identified
from these techniques shall be utilized by assessment and implementing control
measures. Pengawas Keselamatan harus melakukan pemeriksaan harian di lokasi dan
mengidentifikasi potensi bahaya tersebut dan mengambil tindakan korektif langsung.
Sepanjang durasi proyek, teknik identifikasi bahaya harus digunakan. Bahaya dan risiko

Hazard identification shall be conducted prior to commencement of construction for the

reduction of potential major hazards and the preparation of procedures. The objective
being to ensure the risk associated with construction is as low as reasonably practicable.
Identifikasi bahaya harus dilakukan sebelum dimulainya konstruksi untuk pengurangan
potensi bahaya utama dan persiapan prosedur. Tujuannya adalah untuk memastikan
risiko yang terkait dengan konstruksi serendah mungkin.
15.4 Hazardous Material register / Daftar Bahan Berbahaya
A Hazardous Material Register, which will contain Material Safety Data sheets, will be
maintained on the project site for all hazardous materials. This register will be located in
an accessible position to enable all personnel to use it as reference. A Hazardous Material
Register, yang akan berisi lembar Data Keamanan Material, akan disimpan di lokasi

All site supervisors shall continuously monitor work activities to ensure that employees
are adhering to safe work practices. Praise for good safe work practices adopted should
be given as a mean to encourage employee. Identified unsafe practices and
nonconformance should be investigated and corrective action taken. Semua pengawas
lokasi harus terus memantau aktivitas kerja untuk memastikan bahwa karyawan mematuhi
praktik kerja yang aman. Pujian untuk praktik kerja aman yang baik yang diadopsi harus

17.0 Environment / Lingkungan

Before start of the activity an environmental base line study shall be carried out to reflect
the actions to be taken on environmental issues in the course of construction activity. This
shall include direct work related activities, accommodation camp, design,
construction, commissioning and reinstatement. Sebelum memulai kegiatan, studi garis
dasar lingkungan harus dilakukan untuk mencerminkan tindakan yang harus dilakukan
terhadap masalah lingkungan dalam kegiatan konstruksi. Ini harus mencakup kegiatan

17.1 Responsibility / Tanggungjawab
To take ownership of environmental issues environmental factors will be directly taken

care of by work in charge. The work supervisor of each activity shall inspect the area
before start of the job to ensure effectiveness of the system and take corrective action.
An Environmental engineer shall be appointed to manage environmental related issues
during project execution. The recommendation made by Environmental Engineer after
the inspections shall be complied by all department in their area of activity. Untuk
mengambil kepemilikan masalah lingkungan, faktor lingkungan akan langsung ditangani
oleh pekerjaan yang bertanggung jawab. Pengawas pekerjaan dari setiap kegiatan harus
memeriksa area sebelum memulai pekerjaan untuk memastikan keefektifan sistem dan
mengambil tindakan korektif. Insinyur Lingkungan harus ditunjuk untuk mengelola
masalah terkait lingkungan selama pelaksanaan proyek. Rekomendasi yang dibuat oleh
Insinyur Lingkungan setelah inspeksi harus dipatuhi oleh semua departemen di area
aktivitas mereka.
17.2 Compliance of local rules, regulations and procedures / Kepatuhan terhadap

aturan, peraturan dan prosedur setempat
Till the completion of project activity all Indonesian Laws and Regulation shall be
complied, which is highlighted in COMPANY HSENC pertaining to: Sampai dengan
selesainya kegiatan proyek, semua Hukum dan Peraturan Indonesia harus dipenuhi, yang
disoroti dalam HSENC PERUSAHAAN yang berkaitan dengan:

17.4 Noise control / Kontrol Kebisingan

Noise limit in work area will not exceed 85 db during an 8 hour exposure time. This shall
be regularly monitored and necessary action shall be taken time to time to minimize,
reduce and prevent noise exposure to the persons engaged in the activity and the
surrounding area. Adequate no of earmuff and earplug shall be kept at site to use it
wherever it is necessary to wear. This shall include but not limited to the following: Batas
kebisingan di area kerja tidak akan melebihi 85 db selama waktu paparan 8 jam. Ini harus
dipantau secara teratur dan tindakan yang diperlukan harus diambil dari waktu ke waktu
untuk meminimalkan, mengurangi dan mencegah paparan kebisingan kepada orang-orang
yang terlibat dalam kegiatan dan daerah sekitarnya. Tidak ada penutup telinga dan penutup

Case to case study shall be done and appropriate control measures shall be taken to

Noise levels to which personnel are exposed shall be kept as low as reasonably
practicable. Noise exposure levels shall not exceed a peak of 85dB over an 8-
hour working day. Areas around equipments that exceed 85 db(A) shall be clearly marked
and suitable signs posted to remind employees for the use of ear protection. Tingkat

All personnel are required to observe safety signage in designated areas concerning

17.5 Action in the event of spillage / Tindakan jika terjadi tumpahan
 Spilled oil / lubricants shall be initially recovered as much as possible to prevent it

entering in drainage system and to prevent running off. Tumpahan minyak / pelumas
harus dipulihkan sedini mungkin untuk mencegahnya masuk ke dalam sistem drainase
dan untuk mencegah limpasan.
 The person who has seen spillage in first stage will report it to his immediate
supervisor who will immediately rush to the location and arrange to recover the
spillage. Orang yang telah melihat tumpahan pada tahap pertama akan melaporkannya
kepada atasan langsungnya yang akan segera bergegas ke lokasi dan mengatur untuk
memulihkan tumpahan.
 Near spillage location a bund of sand will be provided to prevent runnin g off the
material towards watercourse. Di dekat lokasi tumpahan, sejumlah pasir akan
disediakan untuk mencegah lari dari material menuju aliran air.
 Spilled area shall be immediately cleaned and shall be separately kept in closed
container. Area tumpah harus segera dibersihkan dan harus disimpan secara terpisah
wadah
 Persons engaged in cleaning shall wear hand gloves and other personal protective
equipment to prevent any contact and exposure. Orang yang melakukan pembersihan
harus menggunakan sarung tangan dan perlengkapan pelindung pribadi lainnya untuk
mencegah kontak dan paparan apa pun
 Spilled area will be marked and peoples working there shall not be allowed to enter.
Only personals authorized by supervisor shall be engaged in cleaning. Area tumpah
akan ditandai dan orang yang bekerja di sana tidak akan diizinkan masuk. Hanya
personel yang disahkan oleh supervisor yang akan melakukan pembersihan.
 It shall be the responsibility of supervisor to inform it to HSE dept/administration and
head of section in charge. After receiving the information they shall immediately
rushto site. Merupakan tanggung jawab pengawas untuk menginformasikannya kepada
dept / administrasi HSE dan kepala bagian yang bertanggung jawab. Setelah menerima
informasi, mereka akan segera bergegas ke lokasi.
 Environmental incidence shall be immediately reported to COMPANY. Insiden
lingkungan harus segera dilaporkan ke PERUSAHAAN.
 In the case of oil spill all available information on the spill viz. Cause, time nature,
extent shall be reported to COMPANY. Dalam kasus tumpahan minyak semua
informasi yang tersedia di atas tumpahan yaitu. Penyebab, sifat waktu, jangkauan harus
dilaporkan kepada PERUSAHAAN.
 Detailed investigation shall be done and identified root cause shall be corrected and
monitored regularly to prevent recurrence. Investigasi terperinci harus dilakukan dan
identifikasi akar penyebab harus dikoreksi dan dipantau secara teratur untuk mencegah
kekambuhan.
17.6 Prevention of oil spill in rivers / Pencegahan tumpahan minyak di sungai
All vessels shall take practicable steps to avoid pollution of the sea by oil, chemicals,
and noxious/ hazardous substances. Semua kapal harus mengambil langkah praktis
untuk menghindari pencemaran laut oleh minyak, bahan kimia, dan zat berbahaya /

berbahaya.
Special precautions shall be taken when transferring fuel or other pollutant by hose,
hoses being thoroughly drained prior to disconnection. Fuel is never to be transferred
whilst underway between two vessels. All evidences of pollution, including any observed
slick shall be reported to the COMPANY immediately. Tindakan pencegahan khusus harus
diambil ketika mentransfer bahan bakar atau polutan lainnya dengan selang, selang yang
dikeringkan secara menyeluruh sebelum pemutusan. Bahan bakar tidak pernah ditransfer
ketika sedang berlangsung di antara dua kapal. Semua bukti polusi, termasuk setiap riam yang
diamati harus segera dilaporkan ke PERUSAHAAN.
17.7 Waste management / Pengolahan Limbah
Waste management control system shall manage by work site supervisor; section
Incharge and the ultimate responsibility will lie on project manager. Each department
engaged in the project work shall be responsible and accountable for proper waste
management. As a minimum, the following waste shall be specifications to be included:
Sistem pengendalian pengelolaan limbah harus dikelola oleh pengawas tempat kerja;
bagian Inci dan tanggung jawab utama akan terletak pada manajer proyek. Setiap
departemen yang terlibat dalam pekerjaan proyek harus bertanggung jawab dan
bertanggung jawab atas pengelolaan limbah yang tepat. Minimal, limbah berikut adalah
spesifikasi yang harus disertakan:
 Rubbish, waste, debris, used rags shall be stored in closed metallic containers .

 Waste generated from welding / cutting operation shall be separated from the rest.

 Liquid, Burnable, Non-burnable, Organic, Hazardous substances, Scrap shall be
collected and kept in separate waste bin duly earmarked and colour coded. Bahan-
bahan yang Berbahaya, Terbakar, Tidak Terbakar, Organik, dan Berbahaya, Sisa harus
dikumpulkan dan disimpan di tempat sampah terpisah yang diberi tanda dan diberi
kode warna.
 All Waste generated from project activity shall be properly stored, collected and
disposed to COMPANY approved facilities. Semua limbah yang dihasilkan dari
kegiatan proyek harus disimpan, dikumpulkan dan dibuang dengan benar ke fasilitas
yang disetujui PERUSAHAAN.
 Sufficient no of waste bin shall be located at different location. Cukup tidak ada tempat
sampah harus ditempatkan di lokasi yang berbeda.
 Waste shall be stored in containers and/or areas that have been specially assigned
for waste. They shall be labeled or marked as such. Limbah harus disimpan dalam
wadah dan / atau area yang telah ditetapkan khusus untuk limbah. Mereka harus diberi
label atau ditandai seperti itu.
 No waste shall be disposed into any watercourse or body of water. Tidak ada limbah
dibuang ke setiap aliran air atau badan air.
 All work supervisors shall make Waste Transfer Note to administration department.
The record of the all the Waste Transfer Notes and of all copies issued in a
permanent record. Semua pengawas kerja harus membuat Catatan Transfer Limbah ke
departemen administrasi. Catatan semua Catatan Transfer Limbah dan semua salinan
yang diterbitkan dalam catatan permanen.
 Sufficient no of oil catcher, oil pumps for different grade of oils, measuring shall be
provided at working site. Cukup tidak ada penangkap minyak, pompa oli untuk

berbagai tingkat oli, pengukuran harus disediakan di lokasi kerja.

 All small and big containers shall be provided with its lid. Semua wadah kecil dan
besar harus dilengkapi tutupnya.
 All waste shall container transfer note incorporating type of waste, area of
generation, and quantity for further disposal. Semua limbah harus catatan transfer
kontainer yang menggabungkan jenis limbah, area pembangkitan, dan kuantitas untuk
pembuangan lebih lanjut.
 All hazardous waste such as liquids fuel, hydraulic oils, paints and preservatives
machine /engine filter cartridges batteries shall be segregated and kept separately
and shall be disposed as per recommendation of material safety data sheet. Semua
limbah berbahaya seperti bahan bakar cair, minyak hidraulik, cat dan pengawet mesin
/ cartridge filter mesin baterai harus dipisahkan dan disimpan secara terpisah dan
harus dibuang sesuai rekomendasi lembar data keamanan bahan.
 Waste Management for hazardous and inert waste shall be done in a responsible
manner and complying recommendation of material safety data sheet. Pengelolaan
Limbah untuk limbah berbahaya dan lembam harus dilakukan dengan cara yang
bertanggung jawab dan memenuhi rekomendasi lembar data keamanan bahan.
17.8 Medical waste / Limbah Medis
Medical waste shall be segregated from all other wastes. It shall be collected in
specially designated color-coded containers and disposed at COMPANY approved
hospital. Limbah medis harus dipisahkan dari semua limbah lainnya. Ini harus dikumpulkan
dalam wadah khusus yang diberi kode warna dan dibuang di rumah sakit yang disetujui
PERUSAHAAN.
17.9 Monitoring / Pemantauan
Regular monitoring shall be done to minimize and control the effects on environment and
necessary preventive measures shall be taken during the construction phase. Following
impacts are monitored regularly; Pemantauan teratur harus dilakukan untuk meminimalkan
dan mengendalikan dampak pada lingkungan dan tindakan pencegahan yang diperlukan
harus dilakukan selama tahap konstruksi. Dampak berikut dimonitor secara teratur;

18.0 Emergency planning / Rencana Darurat
To the emergent situations Emergency response plan shall be made and forwarded to
COMPANY before commencement of work for approval. The evacuation plan shall be
required discussed with crew in toolbox talk/pre work briefing. The required emergency
equipment shall be made readily available before commencement of the work i.e.
stretchers at work site, Radio, standby emergency vehicle, first aid box, snakebite kit.
Qualified doctor and medic shall be kept all the time ready for immediate assistance in
the event of injury / illness. Keadaan darurat Rencana tanggap darurat harus dibuat dan
diteruskan ke PERUSAHAAN sebelum dimulainya pekerjaan untuk disetujui. Rencana
evakuasi harus dibicarakan dengan kru dalam pembicaraan bincang-bincang / pra-kerja.
Peralatan darurat yang dibutuhkan harus tersedia sebelum dimulainya pekerjaan, misalnya
usungan di tempat kerja, Radio, kendaraan darurat siaga, kotak pertolongan pertama,
perangkat snakebite. Dokter dan medis yang berkualifikasi harus selalu siap untuk bantuan
segera jika terjadi cedera / sakit.
A detailed emergency response plan shall be prepared and submitted to COMPANY for
approval. All potential emergencies associated with worksite and with the performance
of the work shall cover below mentioned but not limited to following: Rencana tanggap
darurat yang rinci harus disiapkan dan diserahkan kepada PERUSAHAAN untuk disetujui.
Semua potensi keadaan darurat yang terkait dengan tempat kerja dan dengan kinerja pekerjaan
harus mencakup di bawah ini tetapi tidak terbatas pada berikut:

In the event of emergency immediate information shall be given on COMPANY

emergency number, police, and hospital and key personnel of the project. The
CONTRACTOR representative shall be informed who shall immediate take action and
inform key personnel to minimize, control the emergency situation. A list of essential key
personnel and services i.e. fire station, police station and ambulance shall be displayed
on prominent location. Project site management shall be responsible to ensure that the
emergency procedures of the site are properly interfaces and coordinated before the

Evacuation drill shall be conducted as early as practicable at worksite and

accommodates camps. On site emergency situation shall be linked with COMPANY

which will not pose a risk f injury or death or a hazard to the facilities and the plan or
unacceptable impact to the environment and the health and well-being of personnel. In

In the event of oil spill contingency plan shall be made and submitted to COMPANY. The

A separate emergency response plan shall be made for the area managed by
Contractor, which shall interface with Company emergency procedures. Regular training
shall be organized to ensure emergency procedures fully operation. Rencana tanggap

20.0 Storage and Use of Materials / Penyimpanan dan penggunaan Material
20.1 Gas cylinders Tabung gas
All gas cylinders (oxygen, propane, Nitrogen) used for construction purpose shall have
the necessary identification such as: Semua tabung gas (oksigen, propana, Nitrogen) yang

Gas cylinders are to be store in an upright position with the valve protective cover in
place. They are to be segregated according to their content and shall be clearly labeled.

tags shall not to be removed. Adequate ventilation, drainage and retention pits shall be
provided. Transfer of liquid from drums or filling of drums shall be conducted in a safe

valves, vents, and double valve manifold, drain point, quick coupling etc. The
identification of the product and the capacity of tanks shall be clearly displayed on the

complied with. Safety signage, "NO SMOKING", "DANGER AREA" and "HIGHLY
INFLAMMABLE" shall be posted near storage facilities. Natural or forced ventilation
shall be kept to prevent accumulation of hazardous vapor. Classification of hazardous
areas and segregation of hazardous substances shall be notified to avoid ignition and
explosion hazards. No hazardous substances except for quantities necessary for day
use shall be stored in workshops. Particular attention will be paid to the following points.

shall be having access to inspect the storage of radioactive isotopes or when in use. The

 Radioactive materials and ionizing radiation generators shall be handled exclusively

by qualified and appropriately equipped personnel. These personnel shall be known

21.0 Abrasive blasting

Wet blasting shall be performed in preference to dry blasting to minimize air pollution.
This includes any surface preparation on live process equipment. The following safety

system, hand protection, overall suits, apron etc, life jackets, encased hood, and air

vessel, a confined space entry permit shall be available and the following checks shall be

23.2 Welding

 Fireproof items used shall be asbestos free. If any localized heating is to be
done a fire watchman will be stationed at work location. Barang-barang tahan api

pemadam kebakaran akan ditempatkan di lokasi kerja.
 Cylinders of oxygen, propane etc or other flammable gases are never to be
taken inside the confined space where hot works are being done. All cylinders shall
be kept outside in upright and in secured position to prevent falling and impact
exerted during deck operations or from construction equipments, if onshore. Silinder
oksigen, propana dll atau gas mudah terbakar lainnya tidak pernah diambil di dalam
ruang tertutup di mana pekerjaan panas sedang dilakukan. Semua silinder harus
dijaga di luar dalam posisi tegak dan aman untuk mencegah jatuh dan benturan yang

24.0 Pressure Testing / Test tekanan
The recommended test medium is water. Gas (or liquid gases) will be used as a test
medium in case of any specific requirement if recommended by Company. The following
Safety measures shall be adopted during pressure testing. Media uji yang
direkomendasikan adalah air. Gas (atau gas cair) akan digunakan sebagai media uji jika
ada persyaratan khusus jika direkomendasikan oleh Perusahaan. Langkah-langkah

a. All pressurized equipment and fittings shall be of the rated capacity suitable

b. The pressure rating and capacity shall be clearly labeled on all the equipments.

25.0 Electrical safety / Keselamatan Kelistrikan
Access to electrical rooms and servicing of electrical appliances will be restricted to

26.0 Energy supply / Pasokan Energy
In hazardous area wherever possible, CONTRACTOR shall deploy only pneumatically

operated equipments. Compressor and power generators shall be suitable for the
intended use, location and environmental conditions. Generators shall be of diesel
driven only (no petrol engine allowed), equipped with emergency stop controls. Suitable
fire fighting facility shall be located nearby. Only competent persons shall be allowed to
work on power distribution systems. Di daerah berbahaya sedapat mungkin,

pada sistem distribusi tenaga listrik.
All safety devices shall be kept operational and tested according to the manufacturer
specifications and COMPANY requirement. Semua perangkat keselamatan harus dijaga
operasionalnya dan diuji sesuai dengan spesifikasi pabrik dan persyaratan PERUSAHAAN.
27.0 Isolated work locations / Isolasi Lokasi Kerja

The minimum two persons shall be required if working in an isolated or remote location.
Both the persons shall be working in close proximity to each other at all time. They
willhave at least: Minimal dua orang akan diperlukan jika bekerja di lokasi terpencil atau

If work is to be performed at night, adequate illumination shall be provided and a

constant radio watch shall be kept at site. Any break in the communication shall be
immediately investigated and the base radio operator shall be instructed to take

28.0 Safety Equipment / Perlengkapan keselamatan
Contractor will be responsible to provide all site safety equipment associated with site
activities for the project and shall ensure that all safety equipments are maintained in
good working condition as stipulated in the relevant regulations or standards. Kontraktor
akan bertanggung jawab untuk menyediakan semua peralatan keamanan situs yang terkait
dengan kegiatan situs untuk proyek dan harus memastikan bahwa semua peralatan
keselamatan dipelihara dalam kondisi kerja yang baik sebagaimana di atur dalam peraturan
atau standar yang relevan.
29.0 Fall Protection / Perlindungan Jatuh
Any work at a height greater than two meters above ground level or deck level shall
require a fall protection device. Each specific job shall be assessed on its own merit and
appropriate protective mechanisms shall be implemented even When employees work at
height less than two meters. Following action shall be taken for fall protection: Setiap
pekerjaan dengan ketinggian lebih dari dua meter di atas permukaan tanah atau tingkat dek akan
membutuhkan perangkat perlindungan jatuh. Setiap pekerjaan spesifik harus dinilai berdasarkan
kemampuannya sendiri dan mekanisme perlindungan yang sesuai harus diterapkan bahkan ketika
karyawan bekerja dengan ketinggian kurang dari dua meter. Tindakan berikut harus diambil
untuk perlindungan jatuh:
a. Avoid or minimize the work at height by redesign or reschedule. Hindari atau

minimalkan pekerjaan di ketinggian dengan mendesain ulang atau menjadwal ulang.
b. Provide a secure and safe platform i.e. scaffolding / elevated work platform etc
Menyediakan platform yang aman dan aman yaitu perancah / platform kerja yang

c. Prevent falls by providing guardrails and net etc. Mencegah jatuh dengan

e. While working a check shall be done that no loose material is kept at higher

Scaffolding shall be erected only by CERTIFIED person in accordance with the

COMPANY requirements. Scaffolding shall be inspected by COMPANY before being put
into service. Scaffolding shall either have a tag Colour "green (safe) or red (unsafe)". The
sign board shall be written both in English as well as in Bahasa Indonesia. Perancah hanya akan
didirikan oleh ORDERATIF sesuai dengan persyaratan PERUSAHAAN. Perancah harus
diperiksa oleh PERUSAHAAN sebelum dimasukkan ke dalam layanan. Scaffolding harus
memiliki tag Warna "hijau (aman) atau merah (tidak aman)". Papan nama harus ditulis dalam
Bahasa Inggris dan Bahasa Indonesia.

30.0 Personal Protective Equipment (PPE) / Alat Pelindung Diri (APD)
All personnel engaged at work site shall be provided with appropriate Personal
Protective equipment, which shall be monitored regularly by work supervisor for its use
at work locations. The list of PPE for site personnel shall be established in consulta tion
with the COMPANY at the beginning of Project and according to specific requirements.
The signs, signposts and notice board shall clearly indicate the obligation of the
employee to follow PPE requirement according to project conditions. All PPE shall be
checked regularly. Semua personel yang terlibat di tempat kerja harus diberikan peralatan
Pelindung Pribadi yang sesuai, yang harus dipantau secara teratur oleh pengawas kerja untuk
digunakan di lokasi kerja. Daftar APD untuk personel lapangan harus ditetapkan dengan
berkonsultasi dengan PERUSAHAAN pada awal Proyek dan sesuai dengan persyaratan
khusus. Tanda-tanda, papan penunjuk jalan dan papan pengumuman harus secara jelas
menunjukkan kewajiban karyawan untuk mengikuti persyaratan PPE sesuai dengan kondisi
proyek. Semua APD harus diperiksa secara teratur.
30.1 Basic PPE for the project
Please refer to the SOP PPE for more detail & Specific. Silahkan merukjuk pada
SOP PPE untuk lebih detail dan spesifik.
31.0 Medical Fitness / Kebugaran Medis
To ensure positive performance of work and to comply "Health & Safety" policy the
health of the worker shall be given utmost priority. To achieve this all employees shall
undergo a medical checkup prior to assigning them job in the project. Qualified Medical
practitioner shall conduct the medical examinations. Health certificates of all employees
shall be submitted to Company as and when required. Special examinations shall be
carried out for employees whose jobs require them to participate in special certification
categories i.e., security services, vehicle drivers, equipment operators, Welders, NDT
personnel, Blasters, Painters, etc). Administrative Manager shall ensure that regular
health examination of all employee engaged in the project work is done. Medical
examination shall be conducted in accordance with COMPANY requirement. This shall
be done to ensure; Untuk memastikan kinerja positif dari pekerjaan dan untuk mematuhi
kebijakan "Kesehatan & Keselamatan", kesehatan pekerja harus diberikan prioritas
utama. Untuk mencapai ini semua karyawan harus menjalani pemeriksaan medis

yang pekerjaannya mengharuskan mereka untuk berpartisipasi dalam kategori sertifikasi
khusus yaitu, layanan keamanan, pengemudi kendaraan, operator peralatan, Tukang Las,
personil NDT, Blasters, Pelukis, dll.). Manajer Administrasi harus memastikan bahwa
pemeriksaan kesehatan rutin dari semua karyawan yang terlibat dalam pekerjaan proyek
dilakukan. Pemeriksaan medis harus dilakukan sesuai dengan persyaratan
PERUSAHAAN. Ini harus dilakukan untuk memastikan;
 Determine physical capabilities in relation to the essential tasks of the prospective

The number of medical staff mobilized for health care will depend on number of
employees and shall comply COMPANY recommendation reflected in COMPANY
HSENC. The minimum medical staff will be mobilized accordingly. Jumlah staf medis yang

31.1 Medical supply and facility / Persediaan dan fasilitas medis
Medical facilities shall be provided in conformance with COMPANY requirements.

Qualified Doctor, paramedic and medical personnel shall be responsible to run these
facilities. Regular supply of medicine on regular interval shall be done. Adequate
number of Clinic, accommodations for medical staff, adequate equipment & accessories,
and ambulance for routine & emergency care shall be provided. The medical equipment
and medicine procurement shall be done as per list appended in COMPANY HSENC.
Fasilitas medis harus disediakan sesuai dengan persyaratan PERUSAHAAN. Dokter yang
berkualifikasi, paramedis dan tenaga medis harus bertanggung jawab untuk menjalankan
fasilitas ini. Suplai obat secara teratur pada interval reguler harus dilakukan. Jumlah klinik
yang memadai, akomodasi untuk staf medis, peralatan & aksesoris yang memadai, dan
ambulans untuk perawatan rutin & darurat harus disediakan. Pengadaan peralatan medis dan
obat-obatan harus dilakukan sesuai daftar yang tercantum dalam HSENC PERUSAHAAN.
Stock of adequate quantity of medicine shall be all the time maintained for Alimentary
systems, Cardio-vascular system & diuretics, Respiratory system, Neuromuscular
system, Chemotherapeutics (anti-diarrhea, anti-malaria, urinary antiseptics), Nutrition,
Eye, Ear, Mouth & Throat, Dermatological, Anesthesia local & general, Antidotes &

Immunology, Intravenous solution, etc. Stok jumlah obat yang memadai harus selalu dijaga
untuk sistem pencernaan, sistem cardio-vascular & diuretik, sistem pernapasan, sistem
neuromuskuler, kemoterapi (anti-diare, anti malaria, antiseptik kemih), nutrisi, mata, telinga,
mulut & Tenggorokan, Dermatologi, Anestesi lokal & umum, Antidot & Imunologi, Solusi
intravena, dll.
All required and essential medical equipments shall be kept in clinic and at worksite as
per recommendation made in COMPANY HSENC. Semua peralatan medis yang diperlukan
dan penting harus disimpan di klinik dan di tempat kerja sesuai dengan rekomendasi yang
dibuat di HSENC PERUSAHAAN
31.2 First Aid Facility / Fasilitas Pertolongan Pertama
In accommodation camp, project office and at work site first aid facility shall be located
prior to commencement of activity. Trained first aiders shall be deployed to render
immediate medical aid in case of illness/ injury. Sufficient number of first aid kit shall be
kept in the accommodation camp, site offices, and work location, vehicles. The first aid
kit shall be regularly checked and deficient / expired medicines shall be removed and
fresh (not expired) medicine shall be kept in first aid box. Di kamp akomodasi, kantor
proyek dan di tempat kerja fasilitas pertolongan pertama harus ditempatkan sebelum
dimulainya kegiatan. Petugas pertolongan pertama yang terlatih harus dikerahkan untuk
memberikan bantuan medis segera jika terjadi sakit / cedera. Sejumlah alat pertolongan
pertama harus disimpan di kamp akomodasi, kantor lokasi, dan lokasi kerja, kendaraan. Alat
pertolongan pertama harus diperiksa secara teratur dan obat-obatan yang kedaluwarsa /
kadaluarsa harus dikeluarkan dan obat-obatan segar (tidak kadaluwarsa) disimpan di kotak
pertolongan pertama.
31.3 Doctor Assistance in case of Injury / Illness Bantuan / Dokter dalam kasus
Cedera / Penyakit
In case of accidents, medical assistance will be immediately given by the Doctor, Para
Medic, who will immediately attend all minor and major injuries. In serious cases the
injured / patient shall be transferred to / Balikpapan / nearest Hospital by dedicated
emergency vehicle and medic or Doctor shall accompany the injured. Dalam kasus
kecelakaan, bantuan medis akan segera diberikan oleh Dokter, Para Medic, yang akan
segera menghadiri semua cedera ringan dan besar. Dalam kasus serius, korban / pasien
dipindahkan ke / Balikpapan / Rumah Sakit terdekat dengan berdedikasi kendaraan darurat
dan medis atau Dokter harus menemani yang terluka.
31.4 Health Awareness Programme / Program Kesadaran Kesehatan
Regular health awareness program shall be organized to create health awareness

among project employees and workers. This shall cover but not be limited to the

 Clean work environment program shall be organized to ensure Employees fitness
for duty. The HSE department shall monitor it and appraise the matter to project

karyawan yang fit untuk bertugas. Departemen HSE harus memantau dan menilai
masalah tersebut kepada manajemen proyek.
 The work supervisor/ work in charge shall ensure medical surveillance requirements
before the commencement of work and inform Administration Manager for making sure

High standards of personal hygiene are essential at all times. Living in close proximity to a
large number of other people demands cleanliness in order to ensure comfort for
everyone and to prevent spreading of disease. Smoking shall be prohibited in common
mess, offices, at work location and in densely populated residential areas. The smoking
zone shall be designated for the purpose of smoking. Standar kesehatan pribadi yang tinggi

32.1 Meals and Catering / Makanan dan Katering
Meals should ideally not be taken at working places but if unavoidable it shall be taken in
clean and tidy area. If meals have to take place at the work site the barge
superintendent / supervisor shall provide waste bin, hand cleanser and water so that
everyone has the opportunity to wash their hands. Dustbin shall be provided to collect all
food waste. This dustbin should be regularly transferred from work site-to-site camp for
disposal. Makanan idealnya tidak boleh diambil di tempat kerja tetapi jika tidak dapat
dihindari, itu harus diambil di tempat yang bersih dan rapi. Jika makan harus dilakukan di
tempat kerja pengawas / pengawas tongkang harus menyediakan tempat sampah, pembersih
tangan dan air sehingga setiap orang memiliki kesempatan untuk mencuci tangan mereka.

33.0 Alcohol & Drugs / Alkohol & Narkoba
Alcohol and drugs are a potential safety hazard within the workplace and as such it is
forbidden for any person to possess, or be under the influence of alcohol or
nonprescribed drugs, at anytime or anywhere on the work site. Personnel using
prescribed medication shall inform their work supervisor about the drugs, which they
are taking, prior to arrival on site. Alkohol dan obat-obatan adalah bahaya keamanan potensial
di tempat kerja dan karena itu dilarang untuk dimiliki oleh siapapun, atau berada di bawah
pengaruh alkohol atau obat yang tidak diresepkan, kapan saja atau di mana saja di tempat
kerja. Personil yang menggunakan obat yang diresepkan harus menginformasikan pengawas
kerja mereka tentang obat yang mereka pakai, sebelum kedatangan di lokasi.
34.0 Heat stress / Stres Panas
For moderately hot and humid climate which may lead to some personnel experiencing
physical and physiological behavioral changes, following guidelines should be
considered at each project work site for managing worker health and safety in heat
stress conditions. Untuk iklim yang cukup panas dan lembab yang dapat menyebabkan
beberapa personil mengalami perubahan perilaku fisik dan fisiologis, panduan berikut
harus dipertimbangkan di setiap lokasi kerja proyek untuk mengelola kesehatan dan
keselamatan pekerja dalam kondisi tekanan panas.
34.1 Drinking water / Air Minum

Potable and Cool drinking water shall be made readily available at the site for all
personnel exposed to hot conditions. Personnel will be actively encouraged to take
frequent small drinks to replace body fluid lost through sweating. Drinking water supply
shall be based on average of 7-It./person/working day. Water will be supplied
in sealed bottles and the water quality will be tested periodically by independent laboratory.
Air minum yang dapat diminum dan dingin harus tersedia di lokasi untuk semua personil
yang terpapar dengan kondisi panas. Personil akan secara aktif didorong untuk mengambil
minuman kecil yang sering untuk menggantikan cairan tubuh yang hilang karena berkeringat.
Pasokan air minum harus didasarkan pada rata-rata 7-It./person/working day. Air akan
menjadi dipasok dalam botol tertutup dan kualitas air akan diuji secara berkala oleh
laboratorium independen.
34.2 Shade / Shielding / Perlindungan
Where continuous work is required in direct sunlight, provision of shade or shelter shall
be made. It shall work as shelter in case of rain at the site. Di mana diperlukan kerja terus

menerus di bawah sinar matahari langsung, penyediaan naungan atau naungan harus
dilakukan. Ini akan berfungsi sebagai tempat berlindung jika terjadi hujan di lokasi.
34.3 Clothing / Pakaian

The clothing should be as loose fitting as is safe and practicable. Long sleeve shirt shall
be used around working area. Pakaian harus longgar karena aman dan praktis. Kemeja
lengan panjang harus digunakan di sekitar area kerja.
34.4 Working hours / Jam Kerja

Working hours shall not exceed 12 hrs per day. A minimum of eight hours break shall be
maintained between two shifts. Personnel will go for recess during the prescribed
working hour. In extreme weather conditions the work supervisor may re-structure the
work timings to adjust the regular rest breaks. Jam kerja tidak boleh lebih dari 12 jam per
hari. Minimal delapan jam istirahat akan dipertahankan antara dua shift. Personil akan pergi
untuk istirahat selama jam kerja yang ditentukan. Dalam kondisi cuaca ekstrim, supervisor
kerja dapat menyusun ulang waktu kerja untuk menyesuaikan waktu istirahat reguler.
Working in extended hours or working in night shall have prior approval from
COMPANY. All driver, Operators and personnel working in the night shift shall be given
pre work briefing before their assignment to the work on the following points: Bekerja
dalam jam yang diperpanjang atau bekerja di malam hari harus mendapatkan persetujuan
sebelumnya dari PERUSAHAAN. Semua pengemudi, Operator dan personil yang bekerja
di shift malam harus diberikan pengarahan sebelum tugas sebelum pekerjaan dilakukan pada
poin-poin berikut:
 Risk assessment (Job Safety Analysis) for the activity. Penilaian risiko (Analisis
Keselamatan Kerja) untuk kegiatan tersebut.
 Battery limits for work area and access to the area. Batasan baterai untuk area kerja
dan akses ke area tersebut.
 Work shall be stopped in night in a particular location if it is found that it is not
safeto work in dark. Pekerjaan harus dihentikan di malam hari di lokasi tertentu jika
ternyata tidak aman untuk bekerja dalam gelap.
35.0 Work Permit / Ijin Kerja
Work that is required to be covered by a permit shall be obtained as per requirement of

COMPANY. Concerned site supervisor and site safety supervisor shall ensure as follows:
Pekerjaan yang harus dilindungi oleh izin harus diperoleh sesuai persyaratan
PERUSAHAAN. Pengawas situs dan pengawas keamanan situs harus memastikan sebagai
berikut:
 Shall ensure that all work permits are applied/ renewed on time. Akan memastikan
bahwa semua izin kerja diterapkan / diperbarui tepat waktu.
 Shall check and ensure that all work permit (main work permit and complimentary
permits), enclosure such as list of equipment, approved Procedures, risk
assessment, schematic layout of the working area are attached and duly filled
before submission of work permit. Harus memeriksa dan memastikan bahwa semua
izin kerja (izin kerja utama dan izin gratis), lampiran seperti daftar peralatan, Prosedur
yang disetujui, penilaian risiko, tata letak skematik wilayah kerja dilampirkan dan diisi
sepatutnya sebelum pengajuan izin kerja.
 Shall check validity of work permit and apply for extension / new fresh permit as
and when required in consultation with planning department. Harus memeriksa
validitas izin kerja dan mengajukan permohonan perpanjangan / izin baru saat dan bila
diperlukan dalam konsultasi dengan departemen perencanaan.
 Shall retain one copy of work permit and record it and shall give one copy to
planning department for records. Menyimpan satu salinan ijin kerja dan mencatatnya

dan memberikan satu salinan kepada departemen perencanaan untuk catatan.

 Shall monitor that a copy of the work permit(s) is displayed at the worksite at all
times. Harus memantau bahwa salinan izin kerja (s) ditampilkan di tempat kerja setiap
saat.
 Shall maintain work permit register and reflect the status of work permit regularly in
work permit register. Harus mempertahankan daftar izin kerja dan mencerminkan
status izin kerja secara teratur dalam daftar izin kerja.
 Shall inform reflected precautions in work permit to planning, execution,
maintenance and work supervisors for compliance before handing over the work
permit to supervisor. Memberitahukan tindakan pencegahan yang tercermin dalam izin
kerja untuk perencanaan, pelaksanaan, pemeliharaan dan pengawas kerja untuk
kepatuhan sebelum menyerahkan izin kerja kepada supervisor.
 Shall monitor that supervisor in charge of the overall work activity is well aware of
all the tasks associated with the particular jobs. Harus memantau pengawas yang
bertanggung jawab atas aktivitas kerja secara keseluruhan sangat menyadari semua
tugas yang terkait dengan pekerjaan tertentu.
 Shall stop the work if operational condition has been changed and creating
hazardous situation or the persons executing the work are not observing the
precautions. Akan menghentikan pekerjaan jika kondisi operasional telah berubah dan
menciptakan situasi berbahaya atau orang yang melakukan pekerjaan tidak mengamati
tindakan pencegahan.
 Shall regularly close out the work permit after a specified period or after completion of
work. Harus secara teratur menutup ijin kerja setelah jangka waktu tertentu atau
setelah selesai bekerja.
 Shall comply with the work permit procedure. Harus mematuhi prosedur izin kerja.
