•Submit Resignation Letter addressed to the Medical Center Chief II thru the Division Head (Resignation

Letter Form shall be made standardized by the HRMO) to the immediate Supervisor

•shall take note of the resignation letter of the staff and endorse the letter to the Division Head

•upon receipt of the resignation letter, the division head shall write a recommendation (using a standard
Division recommendation form) and endorse the letter to the MCC II

•Accept or Reject the resignation; in case of acceptance, an acceptance letter shall be made
(acceptance letter is a CSC form), printed and signed by the Medical Center Chief and endorse to the
Human Resource Management Office for action

•shall record the employee on the list of employees for separation; shall give the employee the
Human Resource
Clearance Form (clearance form from CSC) and the list of requirements for separation

•comply with the requirements and secure signature from the respective offices for clearance

•check and verify on their list the accountabilities of the employee; immediate supervisor shall secure that
all work responsibilities are properly endorsed to the immediate officer where the outgoing employee is
