CRS Philippines 2014 Typhoon Yolanda Emergency Response

CONTRACT OF WORK
This instrument is entered into by and between CRS Catholic Relief Services a duly
organized Non Government Organization (NGO) and Mr._____________________,of legal
age,married,a resident of Brgy. 3 Quinapondan Eastern Samar hereinafter referred to as Carpenter
for construction of suggestion boxes and bulletin boards.

Both parties agree on the following terms and conditions and responsibilities to wit:

1. The carpenter will work on suggestion boxes and bulletin boards.

2. It is agreed upon that the size of the suggestion boxes is _____________________________
3. It is agreed upon that the size of the bulletin boards is ______________________________
4. It is agreed upon that the carpenter will construct according to CRS approve guidelines and
construction standards and will use all materials towards construction as per guidance of
CRS.
5. It is agreed upon that the carpenter will manage time effectively to complete all the works
within a satisfactory time frame.
6. Will complete his work as quickly and as satisfactorily as required the CRS.
7. Upon the satisfactory completion of the suggestion boxes and bulletin boards CRS will pay
the carpenter.
a. PHP 150.00 for each suggestion boxes .
b. PHP 120.00 for each bulletin boards.
8. CRS will pay the amount above according to CRS records and verification of completion of
the assign works by both CRS and the Protection and Accountability Team.
9. CRS will not pay any extra amount due to delays on the part of the carpenter and/or
unskilled laborer(s) or its partner will not be responsible for any damage or injuries to the
carpenter(s) or unskilled laborer(s) during the construction of the said suggestion boxes and
bulletin boards.
10. Either party has the right to cancel this agreement by giving one week prior notice.

IN WITNESS WHEREOF, the parties hereby signed the above terms and conditions this___day
May 2014. Quinapondan Eastern Samar,Phillipines.
