Information on Project and Proponent; Project Description (including project location and area, project rationale, Proponent files PDR and Application Form project development plan, process and components and and pays the processing fee to the description of project phases, project emissions/ effluent/ Environmental Impact Assessment hazardous wastes/ solid wastes, manpower, project cost, Management Division (EIAMD) project duration and schedule); Overview/Generic Description of the Baseline Environment/ Environmental Management Plan; attachments of Original Sworn Accountability Statement of Proponent and Photos or plates EIAMD, by their EMB Case Handler (EMB of proposed project site, cumulative/residual impact areas CH), reviews the application form and PDR and surrounding communities.

If no decision is made within 4 days (period for
decision-making) from recommendation, the EMB Regional Director approves the CNC application is deemed automatically issuance of the CNC approved and the approving authority shall issue the CNC within five days from the lapse of the period for decision-making
