Dear Mr. San Juan :

Greetings!

This has reference to your request relative to the list of requirements in applying and filling of
National Certificate. The Competency Assessment Operating Procedure (TESDA-OP-CO-05) requires
the following, to wit;

6.1.3 The Processing Officer orients the assessment applicant on the following :
a. Self-Assessment Guide (SAG) (TESDA-OP-QSO-02-F07);
b. Application Form (TESDA-OP-CO-05-F26);
c. Assessment and Certification arrangements;
d. Specification of photo requirement (3 pcs., colored, passport size, (3.5cm x 4.5cm
with head size ranging from 27mm to 31mm ; white background, with collar ; and
with name printed at the back), Mugshot for NCR
e. Other requirements and preparation to be undertaken as may be required in the
Competency Assessment Tools (CATs) ;
f. TESDA approved Assessment Fees ; and
g. Release of NC/COC through courier, if applicable

6.8 Issuance of National Certificate (NC) / Certificate of Competency (COC)

6.8.1 The certified worker presents the Competency Assessment Results Summary
(CARS) to the Provincial Office

6.8.7 The certified worker pays PhP 50.00 certification fee to the cashier prior to the
issuance of certificate

6.12 Authentication of NC or COC (Optional)

6.12.1 The certified worker submits the following requirements for
authentication
a. Two (2) photocopies of NC/COC with documentary stamp
b. payment of PhP 50.00 Authentication Fee for all copies to the
cashier
