SELF-ASSESSMENT SURVEY – HORIZONTAL TYPE – COLLEGE (SAS-HT-C)


Directions: Listed below are characteristics required of colleges as stated in CHED Memorandum Order No. 46, series 2012. Check the box for “Yes” if your institution already
has the required characteristic. If not, check the box for “No.” Then in the column “Details,” cite the statistical data supporting your answer. In the last column, briefly describe
the improvement plans your institution will implement especially for the items you have marked “No”. Attach with this survey form the horizontal instruments which can be
downloaded from the CHED website. Please submit both the survey form and supporting horizontal instruments to the Office of Institutional Quality Assurance and Governance
by December 1, 2016 through the CHED Regional Office. Please submit the SAS form in hard copy and supporting horizontal instruments in soft copies saved in CD. To ensure
the credibility of the answers, this survey form and supporting documents should be duly signed by the president and other appropriate officials.
