OBJECTIVES:
After 1 hour of classroom discussion and demonstration the Level
III nursing students will be able to:

1. Define the following terms:


- Process recording
- Communication
- Non-verbal communication
- Verbal communication
2. Explain the importance of process recording
3. Enumerate the guidelines in process recording
4. Discuss the responsibilities before, during and after
process recording
PROCESS RECORDING
- Is an analysis of an interpersonal interaction.it examines closely the verbal
and nonverbal communication of the nurse-helper and the client seeking help.
- is a detailed written account of the interaction (process) that took place
between the student/worker, client/s or others.
COMMUNICATION
- the imparting or exchanging of information or news.
VERBAL COMMUNICATION
- is the use of sounds and words to express yourself, especially in contrast to
using gestures or mannerisms.
NON-VERBAL COMMUNICATION
- includes facial expressions, the tone and pitch of the voice, gestures
displayed through body language (kinesics) and the physical distance between
the communicators (proxemics).

IMPORTANCE OF PROCESS RECORDING


- To improve written expression.
- To clarify the purpose of interview or intervention.
- To identify strength and weakness.
- To separate facts from self judgment.to establish therapeutic communication
- To evaluate the effectiveness of communication between the client and the nurse
- To facilitate the clients respond to communication process
- To facilitate vital nursing process
- To establish therapeutic communication

GUIDELINES IN PROCESS RECORDING

■ set contract
■ do not touch the client without approval
■ stay out of stroking distance
■ change the topic temporarily if the patient’s behavior escalating
■ remember to keep the focus with the client
■ call for staff assistance if the patient is losing control

RESPONSIBILITIES OF THE NURSE IN PROCESS RECORDING

BEFORE:
 Preparing for process recording:
- define clearly the appropriate objectives to be accomplished regarding nurse patient
interaction.
- learn how to go about in writing a process record
- approach the client in a therapeutic manner
- maintain eye contact
- introduce yourself to the client

DURING:
■ allow the client to talk ( never interrupt the client )
■ use calm and soft voice when talking to the client
■ never agree with the client
■ use therapeutic communication
AFTER:
- set client plan for his/her problem
- recognize good or bad behavior
- documentation
