1. GENERAL
PT Pertamina Balikpapan (here in after referred to as "OWNER") to develop and implement a
Proyek Site Development 2A RDMP RU-V located at PT. PERTAMINA ( PERSERO ),
Balikpapan, East Kalimantan, Indonesia, adjacent to OWNER plants.

2. SCOPE
This Inspection and Test Plan (ITP) provides an inspector with a systematic tool that covers
each phase of electrical work it will likely be performed at Proyek Site Development 2A
RDMP RU-V Balikpapan.

responsible for carrying out activities required
by Contractor.
VENDOR Vendors supplying materials or equipment to
Contractor.
INSPECTION An activity such as measuring, examining or
gauging one or more characteristic of an
entity and compare the results with specified
requirements in order to establish whether
conformity is reached for each characteristic.

H : Hold
Witness the operation test / examination where Contractor’s Inspector and / or
OWNER’s Inspector must be notified in advance and must be present during
the specified activity and no test / examination can be proceeded without
attendance of the related representative(s)
W : Witness
Witness the operation test / examination where the Contractor’s Inspector and /
or OWNER’S Inspector must be notified in advance and shall present during
the specified activity.
SW : Spot Witness
Sample check could be performed for Typicall material. Personnel doing the
work shall notify assigned Quality representative when the work is to be
performed so that witnessing activities could take place. Work does not stop if
assigned representative is not present.
R : Review
Review and acceptance of documents and reports (include films of
radiographic testing).
M : Monitoring
Verifying work and testing activities on an on-going basis as deemed necessary.
