Sir,

As desired we are offer you our AMC charges without spare parts. CRYSTAL SCIENTIFIC
SOLUTION Company takes all the responsibility & proper functioning of the equipment.

TERMS & CONDITIONS OF THE CONTRACT: -

1. Before entering into AMC, it must be ensured thatthe equipment is functional.

2. All maintenance work will have to be carried out within the hospital/Institute premises, If it
is found that particular equipment has to be taken out of the hospital/Institute for repair,
necessary permission from the concerned department must be allowed in writing.
3. The cost of replacement of spare/accessories consumableor any other extra repair
charged extra. We shall submit our estimate for the same for your approval and on only
receipt of your sanction order; we shall depute our service engineer.
4. The contract is deemed effective for a period of 12months, from the date of registration
and receipt of payment.
5. The Annual Maintenance Contract amount, as shown above, is payable, in full and in
advance, alternatively, a mutually agreed between user & contractor (CRYSTAL
SCIENTIFIC SOLUTION).

6. Our service engineer shall provide you three routine service visits and all emergency
breakdown visits during the contract period.
7. The contract does not cover shifting the machine or reinstallation either at the same area
or at a different location. The charges towards re-installation if any shall be made under
our supervision and shall be chargeable extra.
8. At the time of entering in to the Annual Mainten ance Contract, the equipment should be
in perfect working condition.
9. Responsibility for maintenance and servicing of the equipment under Annual
Maintenance Contract shall not be applicable if the equipment is found to have been
tampered with by any person not authorized by CRYSTAL SCIENTIFIC SOLUTION.
10. Preventive maintenance will be provided as per the plan approved at reasonably spaced
intervals to be agreed upon between the CRYSTAL SCIENTIFIC SOLUTION customer.
